THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his record by: * updating his home address * amending the entry in item 7a (Place of Entry into Active Duty) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show Anchorage, Alaska vice Anchorage, Arkansas * showing he is authorized the Meritorious Unit Commendation (MUC) 2. The applicant states his home address needs to be updated and the station location where he joined the service is listed as "Alabama" (sic) when it should be Alaska. He also states he has an award from his second deployment with permanent orders. 3. The applicant provides Permanent Orders 069-34 dated 10 March 2010. CONSIDERATION OF EVIDENCE: 1. On 1 October 2003, the applicant enlisted in the Regular Army at the Anchorage Military Entrance Processing Station, Anchorage, AK. After initial training, he was awarded military occupational specialty 11B (Infantryman). 2. He reenlisted on 22 April 2006. 3. On 17 March 2012, he was honorably discharged. Item 7a of his DD Form 214 shows the entry "Anchorage, Arkansas." The MUC is not shown among the awards listed in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized). Item 19a (Mailing Address After Separation) shows an address in Fort Mitchell, AL. 4. The applicant’s Enlisted Record Brief and Orders 210-7259 shows he was assigned to Company A, 1st Battalion, 5th Infantry Regiment, from 15 September 2008 to 14 September 2009. 5. The applicant provides a copy of Permanent Orders 069-34 from the U.S. Army Human Resources Command (HRC), showing Company A, 1st Battalion, 5th Infantry Regiment, received a MUC for the period of 28 October 2008 to 12 September 2009. 6. The application he submitted to the Board shows his current home address is an address in St. Paul, MN. 7. Army Regulation 635-5 (Separation Documents), in effect at the time, provided the instructions for preparing the DD Form 214. It states the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The regulation states for: a. item 7a, enter the city and state where the Soldier entered active duty; b. item 13, list awards and decorations for all periods of service; and c. item 19a, this address must be a permanent address. The regulation does not provide for updating the entry in item 19a subsequent to discharge. DISCUSSION AND CONCLUSIONS: 1. The applicant’s entered active duty in Anchorage, AK. The entry "Anchorage, Arkansas" in item 7a of his DD Form 214 is clearly an error. 2. The applicant's unit was awarded the MUC for a period during which he was assigned to the unit, and the award is not shown on his DD Form 214. 3. During his discharge processing, he would have provided the information entered in item 19a of his DD Form 214. While his home address may have subsequently changed, there are no provisions for updating this entry following discharge. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to show in: * item 7a – "Anchorage, Alaska" * item 13 – MUC 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
