BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to show:

* award of the Meritorious Service Medal
* completion of the U.S. Army Special Operations Forces Static-Line Jumpmaster Course, 2 weeks, 2011

THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Meritorious Service Medal and jumpmaster qualification.

2.  The applicant states neither his retirement Meritorious Service Medal nor his jumpmaster qualification is shown on his current DD Form 214.

3.  The applicant provides:

* Headquarters, 101st Airborne Division (Air Assault) and Fort Campbell, Orders 027-0617, dated 27 January 2014
* DA Form 638 (Recommendation for Award), dated 15 April 2014
* DA Form 200 (Transmittal Record), dated 11 September 2014
* 3rd Battalion, 5th Special Forces Group (Airborne), Certificate of Graduation

CONSIDERATION OF EVIDENCE:

1.  The applicant was commissioned in the Regular Army on 17 September 2004.

2.  He provided a 3rd Battalion, 5th Special Forces Group (Airborne), Certificate of Graduation, showing he successfully completed the U.S. Army Special Operations Forces Static-Line Jumpmaster Course at Fort Campbell, KY, during the period 25 July 2011 through 11 August 2011 (2 weeks).
3.  Section VI (Military Education) of his DA Form 4037 (Officer Record Brief) shows he completed the U.S. Army Special Operations Command Jumpmaster Course in 2011.

4.  On 23 April 2014, he retired by reason of permanent disability (enhanced).

5.  His DD Form 214 does not show award of the Meritorious Service Medal or completion of the U.S. Army Special Operations Forces Static-Line Jumpmaster Course.

6.  The DA Form 638 he provided shows Headquarters, U.S. Army Special Forces Command (Airborne), Permanent Order Number 129-014, dated 10 June 2014, awarded him the Meritorious Service Medal in connection with his retirement for exceptionally meritorious service during the period 4 May 2004 through 4 May 2014.

REFERENCES:

Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management and provides detailed instructions for preparation of the DD Form 214.  The instructions state to list:

* all Federally recognized awards and decorations for all periods of service
* all formal in-service (full-time attendance) training courses of at least 1 week or 40 hours in duration successfully completed during the period of service covered by the DD Form 214 – include course title, length in weeks, and year completed

DISCUSSION:

1.  The evidence of record shows the applicant successfully completed the U.S. Army Special Operations Forces Static-Line Jumpmaster Course on 11 August 2011 (2 weeks).  His DD Form 214 does not show this course.

2.  The evidence of record shows he was awarded the Meritorious Service Medal in connection with his retirement.  His DD Form 214 does not show this award.
