THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her service in Iraq. 2. The applicant states her post-traumatic stress disorder (PTSD) claim with the Department of Veterans Affairs (VA) was denied because her DD Form 214 does not show she was in the war in Iraq. 3. The applicant did not provide any supporting documentation. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 31 July 2002. She held military occupational specialty 88M (Motor Transport Operator). 2. She was discharged due to pregnancy on 3 August 2004 as a private first class/E-3. She had completed 2 years and 3 days of active duty service. Her DD Form 214 shows in: a. Item 14 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the National Defense Service Medal, Army Service Ribbon, Army Lapel Pin, and Global War on Terrorism Service Medal. b. Item 18 (Remarks): no entry for deployment. 3. Her records contain May 2004 memoranda from her chain of command and the division surgeon from Forward Operating Base Speicher in Iraq, recommending her redeployment to Germany due to pregnancy. 4. An email from the Defense Finance and Accounting Service, dated 17 March 2015, confirmed she received hostile fire pay/imminent danger pay for service in Kuwait from 11 February 2004 through 31 May 2004. 5. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 through 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. The applicant's service in Iraq coincides with the Transition of Iraq (2 May 2003-28 June 2004) campaign. 6. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It states for item 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. Although DFAS's confirmation indicates she was deployed only to Kuwait, evidence shows she was redeployed from Iraq due to pregnancy. As such, she is entitled to correction of her DD Form 214 to show she served in Kuwait/Iraq from 11 February 2004 to 31 May 2004. 2. The available evidence shows she served a qualifying period for award of the Iraq Campaign Medal with one bronze service star. 3. In view of the foregoing, the applicant is entitled to have her records corrected as shown below.
