THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show two awards of the Army Achievement Medal. 2. The applicant states he was awarded the Army Achievement Medal in June 2011 and he received a second award of the Army Achievement Medal in October 2012. However, his DD Form 214 shows only one award of the Army Achievement Medal. 3. The applicant provides copies of a: * DA Form 638 (Recommendation for Award), dated 12 June 2011 * DD Form 214 with an effective date of 24 December 2014 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve (USAR) on 10 December 2009 for a period of 8 years with an active duty service obligation of 6 years. 2. A DD Form 214 shows the applicant entered active duty for training (ADT) on 10 May 2010, was honorably released from ADT on 13 October 2010, and transferred to a USAR unit. He completed the Officer Candidate Course and he was credited with completing 5 months and 4 days of net active service during this period. It also shows in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized): * National Defense Service Medal * Army Service Ribbon 3. On 14 October 2010, he was appointed as a Reserve commissioned officer, in the rank of second lieutenant (2LT)/pay grade O-1, for an indefinite period. 4. A DA Form 638, dated 12 June 2011, shows the applicant was awarded the Army Achievement Medal for meritorious achievement during the period 4 June 2011 to 17 June 2011 announced by Headquarters, 372nd Quartermaster Battalion, Albuquerque Armed Forces Reserve Center, Kirtland Air Force Base, Albuquerque, NM, Permanent Order Number 165-34, dated 20 June 2011. 5. A DA Form 638, dated 9 October 2012, shows the applicant was awarded the Army Achievement Medal for meritorious achievement during the period 13 June 2012 to 4 October 2012 announced by Headquarters, 304th Military Intelligence Battalion, Fort Huachuca, AZ, Permanent Order Number 284-002, dated 11 October 2012. 6. Headquarters, U.S. Army Resources Command, Fort Knox, KY, Orders B-10-206827, dated 28 October 2012, promoted the applicant to first lieutenant (1LT)/pay grade O-2 effective and with a date of rank of 12 October 2012. 7. A DD Form 214 shows the applicant entered ADT on 13 June 2012, was honorably released from ADT on 13 October 2012, and transferred to a USAR unit. He completed the Military Intelligence Basic Officer Course and he was credited with completing 4 months and 1 day of net active service during this period. It also shows in: * item 4a (Grade, Rate or Rank): 2LT * item 4b (Pay Grade): O01 * Item 12 (Record of Service) – * block i (Effective Date of Pay Grade): "2010 10 14" (14 October 2010) * item 13: * Army Achievement Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * German Armed Forces Proficiency Badge Gold 8. A DD Form 214 shows the applicant entered active duty on 30 January 2014, was honorably released from active duty on 24 December 2014 based on completion of required active service, and transferred to a USAR unit. He served in Afghanistan from 12 March 2014 through 21 November 2014. He was credited with completing 10 months and 25 days of net active service during this period. It also shows in item 13: * Afghanistan Campaign Medal with campaign star * Bronze Star Medal * Army Achievement Medal * Meritorious Unit Commendation * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Armed Forces Reserve Medal with "M" Device * Non-Article 5 NATO [North Atlantic Treaty Organization] Medal (International Security Assistance Force) * German Armed Forces Proficiency Badge Gold * German Marksmanship Badge (Shutzenschnur) Silver 9. On 16 December 2015, the applicant was contacted to determine if he wanted his request to include correction of his 13 October 2012 DD Form 214 to show his correct rank/grade (i.e., 1LT/O-2) and the effective date. The applicant concurred with correcting his DD Form 214 to show his correct rank/grade and the effective date. 10. Army Regulation 635-8 (Personnel Separations – Separation Processing and Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army and contains item-by-item instructions for completing the DD Form 214. It shows for: * item 4, enter the active duty grade or rank and pay grade at time of separation * item 12, use extreme care in completing this block since post-service benefits, final pay, retirement credit, etc. are based upon the information contained herein * item 12i, from the most recent promotion order, enter the effective date of promotion to the current pay grade * item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards), each entry will be verified by the Soldier's records DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows permanent orders awarded the applicant the – * Army Achievement Medal for the period 4 June 2011 to 17 June 2011 * Army Achievement Medal (2nd Award) for the period 13 June 2012 to 4 October 2012 a. His 13 October 2012, DD Form 214 and 24 December 2014, DD Form 214 both show only one award of the Army Achievement Medal. b. Based on the evidence of record, both DD Forms 214 should be corrected to show two awards of the Army Achievement Medal. 2. The evidence of record shows the applicant was promoted to 1LT (O-2) effective 12 October 2012. a. His 13 October 2012 DD Form 214 shows the rank/grade of 2LT (O-1) with an effective date of 14 October 2010. b. Based on the evidence of record, his 13 October 2012 DD Form 214 should be corrected to show his correct rank, pay grade, and the effective date. 3. In view of all of the foregoing, it would be appropriate to correct the applicant's DD Forms 214, as recommended below.
