Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 30 November 2014 by removing the entry showing successful completion of the Air War College from item 14 (Military Education). 2. The applicant states, while he attended the Air War College, he retired before completing the course. The entry should be removed. 3. The applicant provides no additional documentary evidence. CONSIDERATION OF EVIDENCE: 1. After having prior enlisted service in the Regular Army, the applicant was commissioned in the U.S. Army Reserve and executed his oath of office on 25 July 1990. He entered active duty on 31 May 1991. 2. In 2002, he executed his oath of office as a commissioned officer in the Regular Army. He served in a variety of positions and rose to the rank of colonel/O-6. 3. He honorably retired on 30 November 2014. Item 14 of his DD Form 214 lists the Air War College, and indicates completion in 2014. 4. His official military personnel file contains a DA Form 1059 (Service School Academic Evaluation Report) which shows he attended the Joint War-Fighting Program, Air War College, during the period 15 July 2013 through 22 005AMay 2014. The form indicates he failed to achieve course standards and was disenrolled with prejudice. 5. Army Regulation 635-8 (Personnel Separations - Separation Processing and Documents) prescribes policies and procedures for the completion of the DD Form 214. With regard to item 14, it states this block should include all formal, in-service training courses successfully completed during the period of service covered by the DD Form 214 (emphasis added). DISCUSSION AND CONCLUSIONS: The evidence of record confirms the applicant was enrolled in the Air War College, but did not successfully complete the training. The applicable regulation states only those course successfully completed should be reflected on the DD Form 214.
