THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service in Afghanistan. 2. The applicant states, in effect: * he was deployed to Afghanistan in support of Operation Enduring Freedom (OEF) from 25 May 2013 to 26 October 2013 * item 18 (Remarks) on his DD Form 214 does not reflect his deployment 3. The applicant provides: * DD Form 214 for the period ending 17 July 2014 * Orders Number GO-130-0001 (A1), dated 20 August 2013, issued by Headquarters, U.S. Army Signal Center of Excellence and Fort Gordon CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a Medical Corps commissioned officer in the U.S. Army Reserve and executed his oath of office on 18 June 2009. He was ordered to active duty on 18 July 2011. 2. He was honorably released from active duty based on having completed his required active service on 17 July 2014. His DD Form 214 shows he completed 3 years of net active service. a. Item 12f (Foreign Service) is blank. b. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) lists: * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon c. Item 18 does not contain any entries showing service in Afghanistan. 3. His official military personnel file (OMPF) contains the following: a. Orders Number GO-130-001, dated 10 May 2013, issued by Headquarters, U.S. Army Signal Center of Excellence and Fort Gordon, showing he was directed to deploy to Afghanistan in a Temporary Change of Station status with a proceed date of 24 May 2013. b. Officer Evaluation Report (OER), for the rated period 24 May 2013 through 25 September 2013, which shows the following entries: * unit of assignment was located in Afghanistan * significant duties indicate he served in a deployed Combat Operational Stress Control Detachment and provided support to 68,000 U.S. and Coalition Forces 4. According to the Defense Finance and Accounting Service (DFAS), the applicant received Hostile Fire/Imminent Danger Pay and Combat Zone Tax Exclusion for the period 2 June 2013 through 28 September 2013 for service in Kuwait. 5. Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and gives guidance for the completion of the DD Form 214. a. Regarding item 12f, it states, to enter the total amount of service performed outside the continental U.S. during the period of the DD Form 214. b. In item 18, show the name of the country to which deployed and the service dates. 6. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF. a. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. b. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns include one for the period 1 July 2011 to 31 December 2014, named Transition I. DISCUSSION AND CONCLUSIONS: 1. The applicant requests his service in Afghanistan be reflected on his DD Form 214 for the period ending 17 July 2014. 2. DFAS shows the applicant served in Kuwait during the period 2 June 2013 through 28 September 2013 (a period of 3 months and 27 days). DFAS is considered the authoritative source when determining foreign service credit based on receipt of hazardous duty pay. Additionally, an OER covering the period 24 May 2013 through 25 September 2013 indicates he was assigned to a unit in Afghanistan during the rated period. Based on this, his DD Form 214 should be amended to show 3 months and 27 days of foreign service and that he served in Kuwait/Afghanistan. 3. He also has qualifying service to be awarded the Afghanistan Campaign Medal with one bronze service star for participation in the Transition I campaign. In item 12f, delete the entry "0000/00/00" and replace it with the entry "0000/03/27." b. Add to item 13 the Afghanistan Campaign Medal with one bronze service star. c.
