THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his request to correct the social security number (SSN) on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge). 2. Applicant states his correct SSN ends with 03XX as is shown on his social security card. 3. Applicant provides – * DD Form 214 * social security card * numerous Ohio Army National Guard (OHARNG) documents CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20140008631, on 7 January 1915. That request was denied because there were no records available except the DD Form 214 in question. 2. The applicant joined the OHARNG on 1 June 1977 using an SSN ending in 03XX. He was ordered to initial active duty for training (IADT) using that same SSN. 3. Upon release from IADT, a DD Form 214 was prepared showing his SSN as ending in 30XX. 4. All of the OHARNG documents submitted by the applicant, including his IADT orders, show an SSN ending in 03XX. REFERENCES: Army Regulation 635-5 (Separation Documents) provides the policy and guidance for preparation of separation documents including the DD Form 214. Historically, this regulation has consistently required that biographical data on the DD Form 214 be taken from the Soldier’s military records. DISCUSSION: 1. The previous application was denied due to the absence of records. 2. The available records now show the applicant served in the OHARNG and attended IADT with the SSN shown on his social security card. 3.
