Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Global War on Terrorism Service Medal (2nd Award) and Defense Superior Service Medal. 2. The applicant states the awards were left off his DD Form 214. 3. The applicant provides Orders SL-357-0033, dated 22 December 2012. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 6 May 2009 and he held military occupational specialty 14T (Patriot Operator/Maintainer). He reenlisted on 1 October 2011. 2. He served in: * Turkey, from 4 January to 7 December 2013 * Afghanistan, from 18 November 2010 to 15 November 2011, in support of Operation Enduring Freedom 3. He was honorably released from active duty on 19 September 2014. His DD Form 214 shows he was awarded or authorized the: * Afghanistan Campaign Medal with 2 bronze service stars * Army Commendation Medal * Army Achievement Medal (2nd Award) * North Atlantic Treaty Organization (NATO) Medal * Meritorious Unit Commendation * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon (3rd Award) * Driver/Mechanic Badge with W/Driver-Wheeled Vehicle Clasp 4. He provides Orders Number SL-357-0033, issued by the U.S. Army Installation Management Command, Fort Sill, OK on 22 December 2012 ordering him to deploy in a temporary change of station to Turkey in support of a NATO Air Defense Artillery mission. REFERENCES: 1. Army Regulation 600-8-22 (Military Awards) provides for award of the Global War on Terrorism Service Medal. This award is awarded or authorized to all Soldiers on active duty, including mobilized Reserve Component Soldiers, on or after 11 September 2001 until a date to be determined and having served 30 consecutive days or 60 nonconsecutive days. Only one award of the Global War on Terrorism Service Medal is authorized. No service stars are prescribed. 2. Military Personnel Message Number 15-084, dated 19 March 2015 authorized Soldiers to wear a service star on the Global War on Terrorism Expeditionary Medal and announced five approved operations as follows: * Operation Enduring Freedom, 11 September 2001 to a date to be determined * Operation Iraqi Freedom, 19 March 2003 to 31 August 2010 * Operation Nomad Shadow, 5 November 2007 to a date to be determined * Operation New Dawn, 1 September 2010 to 31 December 2011 * Operation Inherent Resolve, 15 June 2014 to a date to be determined 3. Army Regulation 600-8-22 also states the Defense Superior Service Medal was established by Executive Order 11904, 6 February 1976. It is awarded by the Secretary of Defense to service members of the Armed Forces of the United States who, after 6 February 1976, rendered superior meritorious service in a position of significant responsibility. The prescribing directive for the Defense Superior Service Medal is DODM 1348.33, Volume 1. DISCUSSION: 1. The applicant was awarded the Global War on Terrorism Service Medal. Only one award of the Global War on Terrorism may be authorized to any individual. A second and/or subsequent awards will not be awarded. 2. There is no evidence the applicant was awarded the Defense Superior Service Medal.
