The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the following to his DD Form 214: a.  block 11 – 11B, Infantryman, 5 years, 8 months, 23 days, b.  block 13 – Meritorious Service Medal (2nd Award), and c.  block 18 – SERVICE IN NIGER 20130802-20131210. 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his: * deployment to Niger from 27 July 2013 to 15 December 2013 in support of Operation Enduring Freedom-Trans Sahara (OEF-TS) * military occupational specialty of 11B (Infantryman) * award of the Meritorious Service Medal (2nd Award) 2. The applicant states: * he deployed to Niger in support of OEF-TS and he needs this shown on his DD Form 214 because he was potentially exposed to radiological material * he entered the Army as an Infantryman in 1995 and he was reclassified as a Supply Specialist in 2001 due to medical reasons * his DD Form 214 does not show his years as an Infantryman * he earned the Meritorious Service Medal (2nd Award) in 2011 and the award was signed after he left the awarding unit and moved to Fort Carson, CO * his award was sent directly to him and it was not added to his Official Military Personnel File 3. The applicant provides: * Enlisted Record Brief * Headquarters, U.S. Army Infantry Center Orders 132-1260, dated 12 May 1995 * Individual Infantry Training Certificate, dated 7 July 1995 * two Meritorious Service Medal Certificates * two DA Forms 638 (Recommendation for Award) * DD Form 1610 (Request and Authorization for Temporary Duty Travel of Department of Defense Personnel) * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 22 March 1995. 2. Headquarters, U.S. Army Infantry Center Orders 132-1260, dated 12 May 1995, shows he was awarded MOS 11B effective 7 July 1995. 3. His DA Form 1059 (Service School Academic Evaluation Report), dated 30 March 2001, shows he completed the Unit Supply Specialist Course during the period 26 January 2001 through 30 March 2001. 4. Headquarters, U.S. Army Special Forces Command (Airborne), Permanent Orders 167-19, dated 16 June 2006, awarded him the Meritorious Service Medal for the period 28 November 2005 to 1 February 2006. 5. Headquarters, U.S. Army John F. Kennedy Special Warfare Center and School, Permanent Orders 262-01, dated 19 September 2011, awarded him the Meritorious Service Medal for the period 1 December 2008 to 15 June 2011. 6. His DD Form 1610, dated 12 July 2013, shows he was issued travel orders for temporary duty in Niamey, Ner (Niger) in support of OEF-TS for the period 27 July 2013 to 15 December 2013. 7. On 31 March 2015, he was retired. His DD Form 214 does not show in: * block 11 (Primary Specialty) – MOS 11B * block 13 (Decorations, Medal, Badges, Citations, and Campaign Ribbons Awarded or Authorized) – Meritorious Service Medal (2nd Award) * block 18 (Remarks) – service in Niger for the period 27 July 2013 to 15 December 2013 8. Section I (Assignment Information) of his Enlisted Record Brief, dated 9 April 2015, shows he was deployed to Niger during the period 2 August 2013 to 10 December 2013. REFERENCES: Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for preparation of the DD Form 214. It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. Instructions stated for: a.  block 11, to enter the titles of all MOSs awarded and held for at least 1 year during the current period of service and include the number of years and months held for each, b.  block 13, to list all federally recognized awards and decorations for all periods of service. Do not enter foreign or State-level awards on the DD Form 214. c.  block 18, to enter Headquarters, Department of the Army mandatory requirements when a separate block is not available and for a continuation of entries. For active duty Soldiers deployed with their unit during their continuous period of active service, enter only, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates)." DISCUSSION: 1. Permanent orders awarded the applicant MOS 11B on 7 July 1995 and he did not complete the Unit Supply Specialist Course until 30 March 2001. His DD Form 214 does not show his award of MOS 11B for the period 7 July 1995 to 29 March 2001. 2. Permanent orders awarded the applicant the Meritorious Service Medal for the periods 28 November 2005 to 1 February 2006 and 1 December 2008 to 15 June 2011. His DD Form 214 does not show his award of the Meritorious Service Medal (2nd Award). 3. Although the applicant contends he was deployed to Niger during the period 27 July 2013 to 15 December 2013 in support of OER-TS, his Enlisted Record Brief shows he was deployed to Niger during the period 2 August 2013 to 10 December 2013.
