Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his missing awards. 2. The applicant states he received additional awards after his DD Form 214 was prepared and signed. 3. The applicant provides a copy of: * Army Good Conduct Medal (AGCM) Order * DA Form 638 (Recommendation for Award) * Army Achievement Medal (AAM) Certificate * DD Form 214 * DD Form 214C (DD Form 214 continuation page) CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 5 June 2000. 2. His record contains a certificate that shows the 563rd Aviation Support Battalion, Fort Campbell, Kentucky, announced his award of the AAM for the period 15 November 2012 to 15 April 2013. The date of the order was not listed on the certificate. 3. On 19 August 2013, the applicant was honorably discharged after completing 13 years, 2 months, and 15 days of net active service this period. Item 13 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded or Authorized) of his DD Form 214 includes the AAM (5th Award) and the AGCM (4th Award). 4. The applicant provides: a. a DA Form 638 showing he was awarded the AAM with 5th Oak Leaf Cluster (OLC) which denotes six awards of the AAM; and b. 563rd Aviation Support Battalion, Permanent Orders Number 296-04, dated 22 October 2012, which announced his award of the AGCM (4th Award). REFERENCES: Army Regulation 600-8-22 (Military Awards) contains the Army's awards policy. Paragraph 3-18 of the awards regulation contains guidance on award of the AAM. It is awarded to any member of the Armed Forces of the United States, or to any member of the Armed Forces of a friendly foreign nation, who distinguished himself or herself by meritorious service or achievement of a lesser degree than required for award of the ARCOM. An OLC is used to denote a second or succeeding awards. DISCUSSION: 1. The applicant was awarded the AAM (6th Award), which is not shown on his DD Form 214. 2. His reason for providing his orders for the AGCM (4th Award) is unclear.
