THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his earlier request for correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Combat Infantryman Badge and two awards of the Army Achievement Medal. As a new issue, he requests correction of his DD Form 214 to show award of the Army Commendation Medal. 2. The applicant states: * these awards are not listed on his DD Form 214 * his unit was awarded the Combat Infantryman Badge, but he never received orders * he deployed to Iraq with the 1st Battalion, 10th Cavalry Regiment * he served in combat * he needs help from the Department of Veterans Affairs 3. The applicant provides: * first page of a DA Form 638 (Recommendation for Award), undated * DD Form 214 CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records in Docket Number AR2011001250 on 10 January 2012. 2. The applicant requests reconsideration of his earlier request for correction of his DD Form 214 to show award of the Combat Infantryman Badge and two awards of the Army Achievement Medal. On 10 January 2012, the Army Board for Correction of Military Records granted relief and a DD Form 215 (Correction to DD Form 214), dated 9 March 2012, amended his DD Form 214 by: a. showing he completed 1 year, 5 months, and 15 days of foreign service; b. adding the following awards: * Army Good Conduct Medal * Army Achievement Medal (2nd Award) * Combat Infantryman Badge * Korea Defense Service Medal * Valorous Unit Award * Iraq Campaign Medal with two bronze service stars * Global War on Terrorism Service Medal c. showing he served in Kuwait/Iraq from 1 April 2003 to 15 September 2003. 3. Since his corrected DD Form 214 shows award of the Army Achievement Medal (2nd Award) (representing two awards) and Combat Infantryman Badge, this portion of his request will not be discussed further in this Record of Proceedings. A copy of the DD Form 215 will be furnished to the applicant. 4. He enlisted in the Regular Army on 4 January 2001. He served in Kuwait/Iraq from 1 April 2003 to 15 September 2003. On 3 January 2004, he was honorably released from active duty. 5. His corrected DD Form 214 shows he was awarded or authorized the: * Army Lapel Button * National Defense Service Medal * Army Service Ribbon * Army Good Conduct Medal * Army Achievement Medal (2nd Award) * Combat Infantryman Badge * Korea Defense Service Medal * Valorous Unit Award * Iraq Campaign Medal with two bronze service stars * Global War on Terrorism Service Medal 6. There are no orders awarding him the Army Commendation Medal in his available records. REFERENCES: Army Regulation 600-8-22 (Military Awards) states the Army Commendation Medal may be awarded to any member of the Armed Forces of the United States who, while serving in any capacity with the Army after 6 December 1941, distinguishes himself or herself by heroism, meritorious achievement, or meritorious service. As with all personal decorations, formal recommendations, approval through the chain of command, and announcement in orders are required.
