THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Afghanistan Campaign Medal and the correct dates of his service in Afghanistan. 2. The applicant states the Afghanistan Campaign Medal is missing from his DD Form 214 and the dates of his foreign service in Afghanistan are wrong. 3. The applicant provides three orders. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 12 March 2012 for a period of 3 years and he held military occupational specialty 11B (Infantryman). He was assigned to C Company, 3rd Battalion, 15th Infantry Regiment, 3rd Infantry Division (ID), Fort Stewart, GA. 2. He deployed to Afghanistan with his assigned unit in support of Operation Enduring Freedom (OEF) in early March 2013. He was wounded in action on 23 July 2013 and was admitted for medical treatment to the Combat Support Hospital, Bagram Airfield, Afghanistan, on that date. 3. On 24 July 2013, he was medically evacuated to Landstuhl Regional Medical Center, Germany. He was subsequently transported back to the United States and on or about 31 July 2013 and he was admitted to Winn Army Community Hospital (WACH), Fort Stewart, GA. On 3 August 2013, he was released from WACH and assigned to the Rear Detachment, 3rd Battalion, 15th Infantry. 3. He was honorably released from active duty on 1 July 2015 and transferred to the U.S. Army Reserve. The DD Form 214 (Certificate of Release or Discharge from Active Duty) he was issued shows the following entries in: a. Item 12f (Foreign Service) - 0000--00--00 b. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the: * Purple Heart * Army Commendation Medal * Army Achievement Medal * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * North Atlantic Treaty Organization (NATO) Medal * Combat Infantryman Badge c. Item 18 (Remarks) - Service in Afghanistan 20130303 - 20130501 4. In an email, dated 23 February 2016, a Defense and Finance Accounting Service (DFAS) official confirmed the applicant received hostile fire (HF)/imminent danger pay (IDP) for service in Afghanistan from 4 March 2013 to 31 July 2013 (150 days or 4 months and 28 days). REFERENCES: 1. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to 31 December 2014. A bronze service star is authorized for wear with this medal for participation in each credited campaign. A review of the applicant's records shows he participated in one campaign (Transition I - 1 July 2011 to 31 December 2014) during his service in Afghanistan. 2. Army Regulation 635-8 (Separation Processing and Documents) states in item 12f of the DD Form 214 enter the total amount of foreign service the Soldier performed outside the continental United States (OCONUS) during the period covered by the DD Form 214. In addition, list periods of deployed service in item 18. DISCUSSION: 1. A DFAS official confirmed the applicant received HF/IDP for service in Afghanistan from 4 March 2013 to 31 July 2013 (4 months and 28 days). However, his DD Form 214 incorrectly shows his service in Afghanistan as 3 March 2013 to 1 May 2013 and item 12f does not show he had 4 months and 28 days of OCONUS service. 2. He served in Afghanistan during a qualifying period for award of the Afghanistan Campaign Medal and participated in one campaign during this service.
