THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Meritorious Service Medal (2nd Award). 2. The applicant states, in effect, his DD Form 214 currently shows only one MSM, because the second MSM was awarded after he was discharged. 3. The applicant provides: * discharge order * DD Form 214 (two copies) * DA Form 638 (Recommendation for Award) * MSM Certificate CONSIDERATION OF EVIDENCE: 1. The applicant's military record shows that after having prior active service and Reserve component service, he entered the Regular Army on 30 October 2005 and continuously served until he was honorably discharged from active duty by reason of disability. 2. Item 13 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded or Authorized) of his DD Form 214 includes an entry for one award of the MSM. 3. He provides a DA Form 638 showing Permanent Order Number 188-01, dated 7 July 2015, from Headquarters, 1st Armored Division, Fort Bliss, TX, awarded the applicant the MSM (2nd Award) for exceptionally meritorious service and performance from 26 April 2014 to 15 July 2015. 4. The applicant's official military personnel file (OMPF) contains the certificate that shows the applicant was awarded the MSM (2nd Award). REFERENCES: Army Regulation 635-5 (Separation Documents) establishes standardized policy for preparing and distributing the DD Form 214. This regulation stipulates that all awards and decorations for all periods of service will be entered on the DD Form 214. Each entry will be verified by the Soldier's records. DISCUSSION: The evidence of record and the evidence provided by the applicant show the Army awarded the applicant the MSM (2nd Award) after he was issued his DD Form 214.
