THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the: * Army Achievement Medal * Army Commendation Medal * Military Outstanding Volunteer Service Medal 2. The applicant states he did not have the award orders/certificates at the time he demobilized. 3. The applicant provides award certificates. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 30 September 1997 and reenlisted on 25 July 2001. He held military occupational specialty 91W (Healthcare Specialist). 2. He was honorably released from active duty on 24 July 2003 and transferred to a troop program unit (TPU) of the U.S. Army Reserve (USAR). He was issued a DD Form 214, which captured his active service. 3. He reenlisted and served in a variety of assignments, including multiple periods of active duty as follows: * 19 March 2004 to 1 April 2005 * 1 October 2009 to 30 September 2010 * 16 November 2010 to 14 November 2012 4. On 18 September 2014, he was ordered to active duty and subsequently served in Honduras from 14 October 2014 to 10 July 2015. He was honorably released from active duty on 11 August 2015.
