The Board determined that the evidence presented was sufficient to warrant partial amendment of the ABCMR's decision in Docket Number AR20140019947, on 28 July 2015. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: * deleting from item 12f of her DD Form 214 the entry "0000-00-00" and adding the entry "0000-05-04" * adding to item 13 of her DD Form 214 the Global War on Terrorism Expeditionary Medal, Iraq Campaign Medal with 2 bronze service stars, and Army Commendation Medal * adding to item 18 of her DD Form 214 the entry "Service in Kuwait/Iraq 20030126 - 20030629" 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of her earlier request for correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her foreign service/deployment to Iraq and associated awards. 2. The applicant states she served in Iraq for 6 months but there is no record of this deployment on her DD Form 214. Her Leave and Earnings Statement (LES) clearly shows she received hostile fire/imminent danger pay (HF/IDP) as well as family separation allowance. 3. The applicant provides: * Post Deployment Health Assessment * Separation orders * March 2003 LES * DD Form 214 * DD Form 215 (Correction to DD Form 214) * DA Form 638 (Recommendation for Award) and certificate for award of the Army Commendation Medal CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20140019947 on 28 July 2015. 2. The applicant provides a Post Deployment Health Assessment, an award recommendation and certificate, and a DD Form 215 which were not previously considered. This warrants consideration by the Board. 3. The applicant enlisted in the Regular Army on 12 January 2000 and she held military occupational specialty 77F (Petroleum Supply Specialist). She was assigned to Hunter Army Air Field, Fort Stewart, GA. 4. She was honorably released from active duty on 20 December 2003. Her DD Form 214 shows she completed 3 years, 11 months, and 9 days of active service. It also shows in: * Item 12f (Foreign Service), zero credit * Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), the National Defense Service Medal, Army Lapel Button, and Army Service Ribbon * Item 18 (Remarks), no entries regarding her service in Iraq 5. On 28 July 2015, the Board reviewed her case but found insufficient evidence to support/confirm that she served in Iraq. The Board, however, determined she was eligible for service awards. 6. On 22 September 2015, she was issued a DD Form 215 (Correction to DD Form 214) that added the Army Good Conduct Medal and Global War on Terrorism Service Medal. 7. An email, dated 11 July 2016, from the Defense Finance and Accounting Service (DFAS) confirmed the applicant received HF/IDP from 27 January through 27 June 2003 for service in Kuwait. 8. She provides her March 2003 LES which confirms she received HF/IDP during this month. She also provides 3 pages of a 4-page Post Deployment Health Assessment, dated 28 June 2003. She indicated that: * she arrived in theater on 27 January 2003 and her date of departure from theater was 29 June 2003 * the location of the operation was in Southwest Asia * she was at Cam Udairi in Kuwait and in Baghdad, Iraq * her deployment was in support of Operation Iraqi Freedom 9. She also provides a DA Form 638 and certificate confirming award of the Army Commendation Medal for service in Iraq in support of Operation Iraqi Freedom from 20 March to 17 April 2003, by Permanent Orders (PO) 148-01, issued by Headquarters, Aviation Brigade, Hunter Army Airfield, GA, on 28 May 2003. REFERENCES: 1. Army Regulation 635-5 (Separation Documents) established the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states for: * Item 12f enter the total amount of foreign service completed during the period covered by the DD Form 214 * item 13, enter all awards, decorations, and badges * item 18 for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "Service in (Name of Country Deployed) From YYYYMMDD - YYYYMMDD)" 2. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. a. The Global War on Terrorism Expeditionary Medal is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in Global War on Terrorism operations on or after 11 September 2001 to a date to be determined. The general area of eligibility (AOE) encompasses all foreign land, water, and air spaces outside the fifty states of the United States and outside 200 nautical miles of the shores of the United States in operations approved by the Secretary of Defense. b. The Iraq Campaign Medal is awarded to members who served in direct support of Operation Iraqi Freedom. The AOE encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns (during his service) are Liberation of Iraq (19 March 2003-1 May 2003) and Transition of Iraq (2 May 2003-28 June 2004). c. Service members qualified for the Global War on Terrorism Expeditionary Medal by reason of service between 19 March 2003 and 28 February 2005, in an area for which the Iraq Campaign Medal was subsequently authorized, will remain qualified for that medal. Upon application, any such service member may be awarded the Iraq Campaign Medal in lieu of the Global War on Terrorism Expeditionary Medal for such service. No service member will be entitled to both medals for the same act, achievement, or period of service. DISCUSSION: 1. The applicant served in Kuwait/Iraq from 26 January 2003 through 29 June 2003, a period of 5 months and 4 days, not 6 months as she contends, as evidenced by her pay records at DFAS and the Post Deployment Health Assessment she completed in connection with her redeployment. Her DD Form 214 does not reflect her foreign service and/or deployment. 2. Based on her service in Kuwait/Iraq, she meets the criteria for award of: * the Global War on Terrorism Expeditionary Medal in that she served in Kuwait from 26 January 2003 to at least 18 March 2003, the beginning of Operation Iraqi Freedom * the Iraq Campaign Medal with two bronze service stars for service in Iraq from on or about 18 March 2003 through 29 June 2003 3. PO awarded her the Army Commendation Medal for service in Iraq in support of Operation Iraqi Freedom.
