Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty), for the period ending 13 October 2015, to show an additional Army Commendation Medal (ARCOM) and to show he completed the Driver's Training Course. 2. The applicant states, in effect, he received the additional ARCOM and completed the Driver's Training Course. For whatever reason, his S-1 did not upload these documents into his official military personnel file (OMPF) prior to his final out appointment. So, the additional ARCOM and training course were not annotated on his original DD Form 214. 3. The applicant provides copies of the following documents: * ARCOM Permanent Orders 256-13, issued by Headquarters, 11th Air Defense Artillery Brigade on 1 November 2012 * ARCOM Permanent Orders 142-01, issued by Headquarters, 69th Air Defense Artillery Brigade on 22 May 2013 * ARCOM Permanent Orders 182-005, issued by Headquarters, [4th] Combat Aviation Brigade, 4th Infantry Division on 1 July 2015 * Certificate of Training, issued by the 4th Combat Aviation Brigade, 4th Infantry Division, Fort Carson, CO on 17 January 2014 CONSIDERATION OF EVIDENCE: 1. The applicant was commissioned as a Regular Army officer on 25 May 2009, in the rank of second lieutenant in the Signal Corps. He entered active duty on 9 June 2009, was promoted to first lieutenant on 23 November 2010, and was promoted to captain on 1 December 2012. 2. The applicant was honorably released from active duty on 13 October 2015. The DD Form 214 he was issued shows in: a. Block 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), he was awarded or authorized the: * ARCOM (2nd Award) * National Defense Service Medal * Global War on Terrorism Expeditionary Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon b. Block 14 (Military Education), the absence of an entry that shows he completed the Driver's Training Course during his period of active service. 3. The applicant provides: a. Permanent Orders 256-13, issued by the 11th Air Defense Artillery Brigade on 1 November 2012, which awarded him the ARCOM (1st Award) for meritorious service during the period 15 November 2011 through 14 November 2012. b. Permanent Orders 142-01, issued by the 69th Air Defense Artillery Brigade on 22 May 2013, which awarded him the ARCOM (2nd Award) for meritorious service during the period 23 March 2010 through 9 June 2013. c. Permanent Orders 182-005, issued by the [4th] Combat Aviation Brigade, 4th Infantry Division on 1 July 2015, which awarded him the ARCOM (3rd Award) for meritorious service during the period 19 December 2013 through 28 July 2015. This award is not shown on his DD Form 214. d. A Certificate of Training, issued by the [4th] Combat Aviation Brigade, 4th Infantry Division on 17 January 2014, which shows he completed the Driver's Training Course during the period 13 January – 17 January 2014 (1 week, a total of 40 hours). REFERENCES: 1. Army Regulation 635-5 (Separation Documents), prescribed the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior duty, and prior inactive duty service at the time of released from active duty, retirement, or discharge (emphasis added). a. Block 13 requires the preparer to list all federally recognized awards and decorations for all periods of service. b. Block 14 requires the preparer to list all formal, in-service (full time attendance) training courses successfully completed during the period of service covered by the 214 of at least one week or 40 hours duration. Include course title, length in weeks, and year completed. This information was intended to assist the Soldier in job placement and counseling; therefore, combat skills were not listed. When in doubt, refer to the American Council of Education’s Guide to the Evaluation of Educational Experiences in the Armed Services for commonly accepted course titles to determine its usefulness to the Soldier after transitioning from the Army. Acceptable source documents include the enlisted record brief, DA Form 4037 (Officer Record Brief), DA Form 1059 (Service School Academic Evaluation Report), or other military issued certificates of completion with from and through dates or number of weeks. DISCUSSION: 1. The applicant's request for correction of his DD Form 214, to show an additional ARCOM and to show he completed the Driver's Training Course, was carefully considered. 2. Permanent Orders 182-005, issued by the [4th] Combat Aviation Brigade, 4th Infantry Division on 1 July 2015, awarded the applicant the ARCOM (3rd Award); however, the ARCOM (3rd Award) is not listed on his DD Form 214. 3. The applicant's Certificate of Training confirms he completed the Driver's Training Course during the period of service covered by his DD Form 214.
