Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her middle name as WXXXX instead of MXXXX. 2. The applicant states there is an error on her DD Form 214 regarding her legal name. She contends that her full legal name since marriage has been K.W.S., not K.M.S. 3. The applicant provides her DD Form 214 and copies of her social security card and driver license. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 23 March 2011. Her DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) shows she listed her full name as K.M.W. 2. A review of her Official Military Personnel File (OMPF) shows that, with the exception of her DD Form 4, she listed her full name as K.M.S. throughout her entire period of active military service. 3. There is no evidence in her OMPF, such as an official court order or marriage license, that indicates she legally changed her name while serving on active duty. Her OMPF is also void of documentation that shows she submitted a request for personnel action requesting a name change. 4. The applicant was discharged on 14 January 2013. Her DD Form 214 shows her full name as K.M.S. Her DD Form 214 also includes an entry in block 18 (Remarks) that indicates she previously listed her name as K.M.W., as shown on her DD Form 4. She signed the DD Form 214 on 10 January 2013 using the name K.M.S. 5. The applicant provided copies of her social security card and driver license, which show the name she now claims is correct. REFERENCES: Army Regulation 635-5 (Separations Documents), in effect at the time of the applicant's active duty service, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It established standardized policy for the preparation of the DD Form 214. It stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. DISCUSSION: 1. The evidence shows that, with the exception of her DD Form 4 in which she listed her full name as K.M.W., she listed her full name as K.M.S. throughout her entire period of active military service. There is no evidence that indicates she ever listed the full name she now claims is correct. 2. For historical purposes, the Army has an interest in maintaining the integrity of its records. The data and information contained in those records should reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, there is a reluctance to recommend that those records be changed. 3. The applicant is advised that a copy of this decisional document will be filed in her OMPF.
