THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her date of enlistment on her DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) to show 14 April 2010. 2. The applicant states her previous DD Form 4 was incorrectly dated in block 5 (Date of Enlistment/Reenlistment) at the Military Entrance Processing Station, Salt Lake City, UT. Block 5 is dated 14 August 2007 and it should read 14 April 2010 as that is when all the signatures were dated in the form. She also requests block 18b (Date signed [biometrically]) to be used as evidence of her actual date of enlistment. The typewritten date in Block 5 should be corrected to align all dates on the documents in order for her to reenlist for another 6 years. 3. The applicant provides: * DA Form 1506 (Statement of Service –For Computation of Length of Service for Pay Purposes), dated 8 May 2007 * Standard Form (SF) 86-1 (Questionnaire for National Security Positions), dated 14 April 2010 * DD Form 4, dated 14 April 2010 * DD Form 1966 (Record of Military Processing – Armed Forces of the United States), dated a 14 April 2010 CONSIDERATION OF EVIDENCE: 1. On 19 May 2001, with no prior military service, the applicant enlisted in the Army Reserve (USAR) for 8 years. She served on active duty for training from 18 July 2001 through 26 October 2001. 2. On 20 June 2004, she was called to active duty to serve as a member of the USAR Control Group Active Guard Reserve (AGR). 3. On 14 August 2007, the applicant was released from active duty and transferred to the USAR Control Group (Reinforcement). 4. On 14 August 2007, the applicant was reassigned from the USAR Control Group (Reinforcement) to a troop program unit. 5. On 18 May 2009, she was honorably discharged from the USAR. 6. On 14 April 2010, the applicant again enlisted in the USAR. The DD Form 4 for this enlistment shows at – * block 5 (Date of Enlistment/Reenlistment) – "20070814." * block 7 (Prior Military Service Upon Enlistment/Reenlistment) – no entries * block 8 (Agreements) – enlistment in the USAR for 6 years in pay grade E-5 * blocks 13 and 14 were biometrically signed on 14 April 2001 * blocks 18 and 19 were biometrically signed on 14 April 2001 7. The applicant reenlisted on 5 March 2016. The DD Form 4 prepared does not include any entry of her prior service. REFERENCES: Army Regulation 140-111 (U.S. Army Reserve Reenlistment Program) states that care is to be taken to ensure that all items are correctly completed without typewriter strikeovers. Any erasures or corrections will be initialed by the reenlistee and the reenlistment official. Errors in the following items may not be erased or corrected. Such errors will require starting over with a new document. * Item 5 (Date of Enlistment/Reenlistment), DD Form 4/1 * Item 8 (Service, period, and pay grade), DD Form 4/1 * Section E, (Confirmation of Enlistment or Reenlistment), DD Form 4/2 DISCUSSION: 1. The DD Form 4 prepared and biometrically signed on 14 April 2010 incorrectly shows the applicant's date of enlistment in block 5 as 14 August 2007. 2. The applicant's available record contains no entry of an enlistment/reenlistment on 14 August 2007. The only action shown on this date was a USAR reassignment.
