The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show: a.  removal of the Korean Service Medal, b.  award of the Army Achievement Medal and Afghanistan Campaign Medal, c.  completion of the Company Intelligence Support Team Course and the Police Intelligence Collector Course, and d.  Missouri instead of Montana as his mailing address after separation. 2. The applicant states an error occurred during the processing of his DD Form 214. 3. The applicant enlisted in the Regular Army on 26 July 2007. He completed training and he was awarded military occupational specialty 31B (Military Police). 2. He provided Certificates of Training showing: a.  He completed the Company Intelligence Support Team Course (40 hours) during the period 13 July 2009 to 17 July 2009 at the U.S. Army Intelligence Center and Fort Huachuca, Fort Huachuca, AZ. b.  He completed the Police Intelligence Collector Course (80 hours) during the period 9 August 2010 to 20 August 2010 at the U.S. Army Military Police School, Fort Leonard Wood, MO. 3. U.S. Army Maneuver Support Center of Excellence Orders LW-299-0033, dated 26 October 2011, show he was deployed in a temporary change of station status in support of Operation Enduring Freedom in Bagram, Afghanistan, effective 6 December 2011 for a period not to exceed 370 days. 4. The Defense Finance and Accounting Service confirmed he received hostile fire pay/imminent danger pay and combat zone tax exclusion for Afghanistan during the period 1 December 2011 through 31 January 2012. 5. He provided a copy of his Army Achievement Medal Certificate that shows 92nd Military Police Battalion, Fort Leonard Wood, MO, awarded him the medal for the period 9 February 2013 to 20 December 2013. 6. On 13 October 2015, he was honorably released from active duty and transferred to the U.S. Army Reserve Control Group (Reinforcement). REFERENCES: 1. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. a.  The Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are: * Liberation of Afghanistan (11 September 2001 – 30 November 2001) * Consolidation I (1 December 2001 – 30 September 2006) * Consolidation II (1 October 2006 – 30 November 2009) * Consolidation III (1 December 2009 – 30 June 2011) * Transition I (1 July 2011 – 31 December 2014) * Transition II (1 January 2015 – date to be determined) b.  The Korean Service Medal is awarded for qualifying service in the theater of operations between 27 June 1950 and 27 July 1954. Qualifying service consisted of permanent assignment, temporary duty for 30 consecutive days or 60 nonconsecutive days, or engaged in active combat against the enemy provided a combat decoration was awarded or an individual certificate was furnished by the commander of an independent force or of a division, ship, or air group, or comparable or higher unit, testifying to such combat credit. 2. Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for preparation of the DD Form 214. It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. Instructions stated for: a.  block 12f, to enter the total amount of service performed outside the continental United States during the period covered in block 12c (Net Active Service This Period). In addition, list periods of deployed service in block 18. b.  block 13, to list all federally recognized awards and decorations for all periods of service. Do not enter foreign or State-level awards on the DD Form 214. c.  block 14, to list all formal, in-service (full time attendance) training courses successfully completed during the period of service covered by the DD Form 214 of at least 1 week or 40 hours duration. (As an exception to full-time attendance, list Command and General Staff College and Senior Service Colleges completed by correspondence courses). Include course title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. d.  block 18, to enter Headquarters, Department of the Army mandatory requirements when a separate block is not available and for a continuation of entries. For active duty Soldiers deployed with their unit during their continuous period of active service, enter only, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates)." e.  block 19a, data source is as provided by the Soldier. Permanent orders awarded the applicant the Army Achievement Medal and this award is not listed on his DD Form 214. 2. The applicant did not serve between 27 June 1950 and 27 July 1954. The Korean Service Medal is erroneously listed on his DD Form 214. 3. He served a qualifying period of service for award of the Afghanistan Campaign Medal with one bronze service star. This award is not listed on his DD Form 214. 4. His deployed service to Afghanistan in support of OEF for the period 1 December 2011 to 31 January 2012 is not listed on his DD Form 214 and this period of 62 days was not added to his total Foreign Service credit on his DD Form 214. 5. He completed the Company Intelligence Support Team Course during the period 13 July 2009 to 17 July 2009 and the Police Intelligence Collector Course during the period 9 August 2010 to 20 August 2010. These courses are not listed on his DD Form 214. 6.
