Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant's fiancée, using a power of attorney, requests correction of item 19a (Mailing Address After Separation) of the applicant's DD Form 214 (Certificate of Release or Discharge from Active Duty) from "10XX Tr_ Ct, Lebanon, OH 45036" to "8XX Ox__ Drive, Dahlonega, GA 30533." 2. The applicant states the address shown on his DD Form 214 was the address when he enlisted. He no longer resides there. His fiancée is signing the documents because he was overseas at the time this form was submitted. She has a power of attorney over his affairs. 3. The applicant provides: * Check from Amicalola Electric Membership Corporation * Durable Power of Attorney CONSIDERATION OF EVIDENCE: 1. The applicant's records show he enlisted in the Regular Army on 14 September 2009. His DD Form 4 (Enlistment/Reenlistment Document) listed his home of record as "10XX Tr_ Ct, Lebanon, OH 45036." 2. He held an infantry military occupational specialty. He served in a variety of stateside or overseas assignments and he attained the rank of sergeant/E-5. 3. On 23 May 2013, he was assigned to the Ranger Training Battalion, Dahlonega, GA. 4. Headquarters, U.S. Army Garrison, Fort Benning, GA, published Orders 064-2248 reassigning him to the Transition Center for separation processing effective 7 June 2015. The orders listed his home of record as Lebanon, OH. 5. He was honorably released from active duty on 7 June 2015. His DD Form 214 shows in item 19a, "10XX Tr_ Ct, Lebanon, OH 45036." 6. He provides a check from Amicalola Electric Membership Corporation, dated 8 December 2015, listing his address as "8XX Ox__ Drive, Dahlonega, GA 30533." REFERENCE: Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management and separation document preparation. The DD Form 214 is a summary of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of separation from active duty. The DD Form 214 is not intended to have any legal effect on termination of a Soldier’s service. Paragraph 5-6 provides for detailed instructions for data required in each block of the DD Form 214. Item 19 provides the contact Address Information: * Item 19a, Mailing Address After Separation; the data source is as provided by the Soldier; this address must be a permanent address; enter street address, city, state, and zip code * Item 19b, Nearest Relative; the date source is as provided by the Soldier; the name and address of a relative should be someone who will know Soldier’s location and address at all times DISCUSSION: 1. The DD Form 214 captures a member's most recent period of continuous active duty and provides a record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. 2. The entry pertaining to the mailing address after separation is provided by the Soldier to the official preparing the DD Form 214. The applicant appears to have provided the permanent mailing address that is shown in item 19a of his DD Form 214. 3.
