THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty), for the period ending 29 August 2009 to add multiple orders. 2. The applicant states that she would like to add all orders to her DD Form 214. These orders were never added to her DD Form 214. She is a Reserve Soldier and did not know she needed to out process at a base when the orders ended. 3. The applicant provides multiple orders (with different dates) and her DD Form 214. CONSIDERATION OF EVIDENCE: 1. On 17 November 2008, the applicant enlisted in the United States Army Reserve (USAR) for a period of 8 years. 2. On 3 March 2009, she entered active duty for training. She was released from active duty training on 29 August 2009. Her DD Form 214 shows she completed 5 months and 27 days of total active service. 3. The applicant’s Chronological Statement of Retirements Points does not provide the actual dates of when she entered and completed her active duty tours of 90 days or more. 4. The applicant provided only two orders of 90 days or more. REFERENCES: Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge, and is not intended to have any legal effect on termination of a Soldier's service. A DD Form 214 will be prepared for each Soldier as indicated: a. Active Army Soldiers on termination of active duty by reason of administrative separation (including separation by reason of retirement or expiration of term of service), physical disability separation, or punitive discharge under the Uniform Code of Military Justice; b. Reserve Component (RC) Soldiers completing 90 days or more of continuous active duty for training (ADT), Full-Time National Guard Duty, active duty for special work, temporary tours of active duty, or Active Guard/Reserve (AGR) service. Also, RC Soldiers separated for cause or physical disability, regardless of the length of time served on active duty; c. RC Soldiers completing initial ADT that results in the award of a military occupational specialty, even when the active duty period was less than 90 days. This includes completion of advanced individual training under the ARNG of the United States Alternate Training Program or the USAR Split Training Program. DISCUSSION: 1. The available records show the applicant served on active duty from 3 March 2009 to 29 August 2009, and she was issued a DD Form 214 that captured this period of active duty. 2. Governing regulations authorize a DD Form 214 for Reserve Component Soldiers completing 90 days or more of continuous active duty for training, full-time National Guard Duty, active duty for special work, temporary tours of active duty, or Active Guard/Reserve service. Also, RC Soldiers separated for cause or physical disability, regardless of the length of time served on active duty. 3. The applicant provided copies of multiple orders that ordered her to active duty and show various numbers of duty days; however, based on the records available to the Board, it is not clear and there is no way to verify when she actually entered active duty or completed the tour of active duty. 4. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge.
