THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to reflect his deployment to Afghanistan from 15 April 2012 through 15 January 2013 and award of the Afghanistan Campaign Medal. 2. The applicant states his DD Form 214 lacks this information and is preventing him from receiving the benefits he is due. 3. The applicant provides: * Installation Management Command, Joint Base Lewis-McChord, Orders LS-072-0004, dated 12 March 2012 * Combined Task Force Lancer, 2d Stryker Brigade Combat Team, 2d Infantry Division, Permanent Orders Number 194-4, dated 12 July 2012 * DA Form 638 (Recommendation for Award), dated 14 August 2012, showing 2d Stryker Brigade Combat Team, 2d Infantry Division, Permanent Order Number 259-119, dated 3 October 2012 * Certificate of Achievement, dated 30 November 2012 * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 25 October 2010. 2. Installation Management Command, Joint Base Lewis-McChord, Orders LS-072-0004, dated 12 March 2012, deployed the applicant in a temporary change of station status to Kandahar, Afghanistan, in support of Operation Enduring Freedom (OEF) on or about 15 April 2012. 3. Combined Task Force Lancer, 2d Stryker Brigade Combat Team, 2d Infantry Division, Permanent Orders Number 194-4, dated 12 July 2012, awarded him the Combat Action Badge effective 21 June 2012. 4. 2d Stryker Brigade Combat Team, 2d Infantry Division, Permanent Order Number 259-119, dated 3 October 2012, awarded him the Army Commendation Medal for exceptionally meritorious service during combat operations in Afghanistan for the period 27 April 2012 to 17 January 2013. 5. He provided a Security Force Assistance Team Shah Wali Kot District Headquarters Certificate of Achievement, dated 30 November 2012, which is partially illegible, but appears to have been presented for meritorious service in Afghanistan. 6. On 3 April 2014, he was released from active duty. The Defense Finance and Accounting Service reviewed his Master Military Pay Account and verified he received hostile fire pay/imminent danger pay for service in Afghanistan from 1 April 2012 through 12 January 2013, a period of 9 months and 11 days. REFERENCES: 1. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. The Afghanistan Campaign Medal is awarded to members who served in direct support of OEF. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are: * Liberation of Afghanistan (11 September 2001-30 November 2001) * Consolidation I (1 December 2001-30 September 2006) * Consolidation II (1 October 2006-30 November 2009) * Consolidation III (1 December 2009-30 June 2011) * Transition I (1 July 2011-31 December 2014) * Transition II (1 January 2015-date to be determined) 2. Army Regulation 635-5 (Separation Documents), in effect at the time, established standardized policy for preparing and distributing the DD Form 214. This regulation stated the purpose of the separation document was to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement, or discharge. It is important that information entered on the form is complete and accurate and reflects the conditions as they existed at the time of separation. The instructions stated: a. enter the total amount of foreign service completed during the period covered in item 12f; b. list the decorations, medals, badges, citations, and campaign ribbons for all periods of service in the priority sequence specified in Army Regulation  600-8-22 in item 13; and c. for an active duty Soldier deployed with his or her unit during their continuous period of active duty, enter the statement: "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" in item 18. DISCUSSION: 1. The evidence of record shows he deployed to Afghanistan from 1 April 2012 through 12 January 2013, a period of 9 months and 11 days. This period of foreign service is not annotated in item 12f or item 18 of his DD Form 214. 2. He served a qualifying period for award of the Afghanistan Campaign Medal with one bronze service star.
