The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of the previous Army Board for Correction of Military Records (ABCMR) decision set forth in Docket Number AR20140018399 on 9 June 2014. Specifically, he requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) for the period ending 30 June 1971 to show his date of entry as 6 June 1967 instead of 17 March 1971 and to show a different home of record. 2. The applicant states he entered the Army on 6 June 1967 and the address listed on his DD Form 214 is wrong; he has never lived at the address listed. He was 16 years old when he entered the Army. He has moved all over the United States and has lost every record he ever had. 3. The applicant provides a 3-paged self-authored statement and the following: * DD Form 4 (Enlistment Contract – Armed Forces of the United States), documenting his enlistment in the U.S. Marine Corps (USMC) on 26 June 1969 * NAVMC Form 118(11) (Administrative Remarks (1070)), with altered and illegible dates * a roster of unknown origin, titled "Company B Vietnam," dated 7 July 2012 * an undated, self-authored note * a DD Form 214, for the period ending 23 July 1969 * a DD Form 214, for the period ending 30 June 1971 CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records that were summarized in the previous consideration of the applicant's case by the ABCMR in Docket Number AR20140018399 on 9 June 2014. 2. As new evidence, the applicant provides a roster titled "Company B Vietnam" and documents that indicate he served in the USMC prior to enlisting in the Army. These documents were not considered in the applicant's initial case; therefore, they warrant consideration at this time. 3. A DD Form 4, dated 17 March 1971 shows the applicant enlisted in the Regular Army on 17 March 1971, for 3 years. The DD Form 4 shows in: * item 21 (Date of Birth) - 15 May 1951 * item 41 (Home of Record) shows an address on Logan Boulevard in Chicago, IL * item 49 (Prior Service) does not show he had any prior service 4. In conjunction with his enlistment, the applicant completed and signed a DD Form 398 (Statement of Personal History) which shows in: * item 4 (Permanent Mailing Address) shows an address on Logan Boulevard in Chicago, IL * item 8 (Military Service) does not show he had any prior service 5. The applicant was discharged on 30 June 1971. The DD Form 214 he was issued bears his signature, and shows in: * item 9 (Date of Birth) - 15 May 1951 * item 11d (Effective Date) - 30 Jun 1971 * item 17c (Date of Entry) - 17 March 1971 * item 21 (Home of Record at Time of Entry into Active Service) an address on Logan Boulevard in Chicago * item 22a(2) (Other Service) - None * item 22c (Foreign and/or Sea Service) - None 6. The applicant's official military record is void of evidence that shows he served in the USMC or that shows he served in the Republic of Vietnam. 7. The applicant provides: a. An undated, self-authored statement, wherein he states he got into some trouble in May 1966 and had to appear in court. The judge gave him the choice of going into the Army or to jail. He chose the Army, his parents went with him to sign the papers. On 24 May 1966, he went back to court and was cleared of any wrong doing. He signed a contract that he could wait until he graduated from high school. On 6 June 1967, he left for Fort Lewis, WA and completed 8 weeks of basic training. He graduated from basic training on 24 July 1967. b. He describes an attack against the North Vietnamese, on 28 February 1968, wherein he became a one-man assault team and crawled through 75 meters of intense hostile fire exposing himself to the enemy, to keep everyone in his unit safe. He lives that war 24 hours a day, 7 days a week, and 52 weeks a year. c. A DD Form 214, for the period ending 23 July 1969, which indicates he served in the USMC for 27 days and 16 days of other service, totaling 1 month and 13 days. d. A roster titled "Company B Vietnam", dated 7 July 2012. REFERENCES: Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. a. It establishes standardized policy for the preparation of the DD Form 214. b. It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active service at the time of release from active duty, retirement, or discharge. c. It provides instructions for the following: * item 11d - enter the date separation is accomplished * item 17c - enter the date entered on active duty or the date of enlistment or reenlistment was accomplished * item 21 - enter the home of record at the time of entry into active duty as shown on enlistment record, regardless of place physically located at time of entry on active duty DISCUSSION: 1. The applicant's request for correction of his DD Form 214, for the period ending 30 June 1971, to show he entered the Army on 6 June 1967 and he entered from a different home of record, was carefully considered. 2. The applicant provides a DD Form 214 for the period ending 23 July 1969, which shows he served honorably in the USMC from 26 June to 23 July 1969, a period of 27 days. 3. The evidence of record shows the applicant enlisted in the Regular Army on 17 March 1971 and was discharged on 30 June 1971, prior to completion of basic combat training. 4. The applicant provides an enlistment contract that differs from the original contract contained in his official military record. Additionally, he contends he served in the Republic of Vietnam; however, the available records provide no evidence to support this contention. 5.
