THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 26 April 2013 to show his foreign service in Afghanistan. 2. The applicant states his DD Form 214 has no deployment dates; he needs this corrected. He was deployed on 19 October 2011 to Afghanistan. 3. The applicant provides: * Orders 45-3219, issued by Headquarters, U.S. Army Maneuver Center of Excellence, Fort Benning, GA on 14 February 2011 * In processing checklist from 3rd Battalion, 509th Infantry Regiment, dated 31 March 2011 * Orders RI-292-0530, issued by U.S. Army Element, Joint Base Elmendorf-Richardson on 19 October 2011 * North Atlantic Treaty Organization (NATO) Medal Certificate * DA Form 4980-14 (Army Commendation Medal (ARCOM) Certificate), annotated with Permanent Order Number 253-089, issued by Task Force Spartan, Forward Operating Base Salerno on 9 September 2012 * DD Form 214 for the period ending 26 April 2013 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 12 October 2010. He completed his initial entry training and was awarded military occupational specialty 11B (Infantryman). 2. Defense Finance and Accounting Service (DFAS) records confirm he received hostile fire pay/imminent danger pay (HFP/IDP) and combat zone tax exclusion (CZTE) for service in Kuwait and Afghanistan during the period 1 December 2011 through 31 October 2012, a period of 11 months and 1 day. 3. The applicant's DD Form 214 shows he was discharged on 26 April 2013. His DD Form 214 does not show any foreign service credit nor any entries that indicate he deployed in support of any named operations. Block 13 (decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) shows he was awarded or authorized the National Defense Service Medal, the Global War on Terrorism Service Medal, the Army Service Ribbon, and the Parachutist Badge. 4. The applicant provides: a. A NATO Medal Certificate that shows he was authorized the NATO Medal for service with NATO in relation to the International Security Assistance Force (ISAF) Operation, during the period 15 December 2011 through 10 October 2012. b. An ARCOM Certificate showing Permanent Orders 253-089, issued by Task Force Spartan, Forward Observation Base Salerno, Afghanistan on 9 September 2012, awarded him the ARCOM for meritorious service while deployed in support of Operation Enduring Freedom (OEF) 12-13. REFERENCES: 1. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. During his service in Afghanistan, participation credit was awarded for the following campaign: Transition I (1 July 2011 – 31 December 2014). 2. Army Regulation 635-5 (Separation Documents) prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It provided standardized policy for the preparation of the DD Form 214. Preparers of the DD Form 214 will use Block 12f to enter the total amount of service performed outside the continental United States during the period. Preparers of the DD Form 214 will use block 18 to show an entry for an active duty Soldier who deployed to a foreign country with his or her unit during their continuous period of active service by entering the statement "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)." DISCUSSION: 1. DFAS records confirm the applicant received HF/IDP and CZTE for service in Kuwait and Afghanistan from 1 December 2011 through 31 October 2012. However, this period of foreign service and its location were not entered on his DD Form 214 as required by regulation. 2. Based on the applicant’s service in Afghanistan, he participated in one campaign. As authorized by regulation, his DD Form 214 should reflect the Afghanistan Campaign Medal with one bronze service star in recognition of his campaign participation. 3. Permanent orders awarded the applicant the ARCOM; however, this award is not shown on his DD Form 214. 4.
