THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) by showing he was deployed to Haiti. 2. The applicant states he was deployed to Haiti from 30 May to 2 November 1995; however, this service was not entered in Item 18 (Remarks) of his DD Form 214. 3. The applicant provides: * DA Form 638 (Recommendation for Award) dated 7 September 1995 * DA Form 4980-14 (Certificate for Army Commendation Medal) dated 18 September 1995 * DA Form 67-8 (U.S. Army Officer Evaluation Report), dated 16 November 1995 * DD Form 214 for the period ending on 30 June 2014 with DD Form 214C (Continuation Sheet) CONSIDERATION OF EVIDENCE: 1. On 21 May 1994, the applicant entered the Regular Army as a second lieutenant. 2. DA Form 67-8, as provided by the applicant, shows on or about 19 November 1994, the applicant was assigned to B Troop, 1st Squadron, 2nd Armored Cavalry Regiment, for duty as a platoon leader. 3. DA Form 638, as provided by the applicant, shows he was recommended for award of the Army Commendation Medal for his achievements while deployed to Haiti from 30 May to 2 November 1995. This award was announced in Permanent Order Number 76-28, dated 18 September 1995. 4. DA Form 4980-14, dated 18 September 1995, as provided by the applicant, states in the citation the applicant was assigned in support of the United Nations Mission in Haiti from 30 May to 2 November 1995 and he performed his duties meritoriously. 5. On 30 June 2014, the applicant was retired due to having completed sufficient service. He had attained the rank of lieutenant colonel, pay grade O5. His DD Form 214, to include the DD Form 214C, does not show his deployment to Haiti in 1995. 6. Records at DFAS show the applicant served in Haiti from 1 May to 30 November 1995. (This data is based on eligibility for special pay related to deployment, which is paid in whole months even if a Soldier is deployed for only a portion of the month.) 7. A DA Form 4037 (Officer Record Brief), dated 25 February 2014, shows the applicant was deployed to Haiti for 5 months ending on 1 November 1995. REFERENCES: Army Regulation 635-5 (Separation Documents), in effect at the time, provided detailed instructions for completing separation documents, including the DD Form 214. It states for item 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION: 1. The applicant contends his DD Form 214 should be corrected by showing he was deployed to Haiti from 30 May to 2 November 1995. 2.
