Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 19 April 2015 to show a second award of the National Defense Service Medal (NDSM). 2. The applicant states he is requesting a bronze service star for his NDSM. He is also requesting that his Officer Record Brief (ORB) reflect the change. He served in the U.S. Army Reserve (USAR) during the Persian Gulf War and served on active duty during Operation Iraqi Freedom/Operation Enduring Freedom. 3. The applicant provides his DD Form 214, ORB, and DA Form 2-1 (Personnel Qualification Record). CONSIDERATION OF EVIDENCE: 1. After having prior enlisted service in the USAR and Army National Guard (ARNG), the applicant was appointed as a Reserve commissioned officer in the rank of second lieutenant on 27 September 2006. He is presently serving as an Army nurse in the rank of major. 2. His record contains a DA Form 2-1 (Personnel Qualification Record – Part II) that shows in item 35 (Record of Assignments) the applicant was assigned as follows: * USAR troop program unit from 6 November 1989 to 8 September 1993 * ARNG from 9 September 1993 to 1 August 1996 * USAR control group (annual training) from 2 August 1996 to 26 August 1997 3. Item 9 (Awards, Decorations, and Campaigns) of his DA Form 2-1 shows he was awarded or authorized the NDSM. 4. His record contains a DD Form 214 that shows, on 19 April 2015, he was honorably REFRAD after completing 3 years of total active service. His DD Form 214 shows only one NDSM. REFERENCE: Army Regulation 600-8-22 (Military Awards) states: a. The NDSM is awarded for honorable active service for any period between 27 June 1950 and 27 July 1954, 1 January 1961 and 14 August 1974, 2 August 1990 and 30 November 1995, and 11 September 2001 and a date to be determined. Executive Order 12776 extended award of the NDSM to all members of the Army National Guard and the U. S. Army Reserve who were part of the Selected Reserve in good standing during the period 2 August 1990 through 30 November 1995. b. Service stars are worn on campaign and service medals to denote participation in a named campaign and on service ribbons (for example the NDSM) to denote an additional award. DISCUSSION: The evidence of record shows the applicant served during two qualifying periods for which the NDSM was authorized. His DD Form 214 for the period ending 19 April 2015 shows he was awarded or authorized only one NDSM.
