THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show: * two awards of the Army Commendation Medal * two awards of the Army Achievement Medal 2. The applicant states upon his discharge from active duty, he was supposed to receive the Army Commendation Medal. However, the award was downgraded to an Army Achievement Medal. In effect, his DD Form 214 shows three awards of the Army Commendation Medal and one award of the Army Achievement Medal. 3. The applicant provides: * DD Form 214 * Enlisted Record Brief * Army Commendation Medal Certificate (Permanent Orders (PO) 317-006) * Army Commendation Medal Certificate (PO Number 120-167) * DA Form 638 (Recommendation for Award) and Army Achievement Medal Certificate (PO Number 281-1) * DA Form 638 and Army Achievement Medal Certificate (PO Number 093- 04) CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 3 August 2006 and he held military occupational specialty 92Y (Unit Supply Specialist). He served in Iraq: * from 6 July 2010 to 20 June 2011 * from 28 October 2007 to 20 December 2008; he was assigned to the 1st Battalion, 76th Field Artillery 2. He was honorably discharged on 12 September 2014. His DD Form 214 shows he was awarded or authorized the: * Iraq Campaign Medal with 3 bronze service stars * Army Commendation Medal (3rd Award) * Army Achievement Medal * Army Good Conduct Medal (2nd Award) * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon (2nd Award) * Certificate of Achievement * Driver and Mechanic Badge with Driver-Wheeled Vehicle Clasp 3. PO Number 317-006, issued by Headquarters, 4th Brigade, 3rd Infantry Division, on 12 November 2008, awarded him the Army Commendation Medal for service in support of Operation Iraqi Freedom from 24 October 2007 to 10 January 2009. 4. PO Number 255-01, issued by Headquarters, U.S. Army Human Resources Command, on 12 September 2011, awarded his unit (1st Battalion, 76th Field Artillery) the Meritorious Unit Commendation for meritorious service from 10 July 2010 to 21 June 2011. 5. PO Number 120-167, issued by Headquarters, 4th Brigade, 3rd Infantry Division, on 29 April 2011, awarded him the Army Commendation Medal for service in support of Operation Iraqi Freedom from 10 July 2010 to 21 June 2011. 6. PO Number 093-04, issued by Headquarters, 4th Battalion, 10th Infantry Regiment, Fort Jackson, SC on 4 April 2014, awarded him the Army Achievement Medal for achievement from 13 March to 4 April 2014. 7. PO Number 281-1, issued by Headquarters, 2nd Battalion, 39th Infantry Regiment, Fort Jackson, on 8 October 2014, awarded him the Army Achievement Medal for service from 2 April 2013 to 16 September 2014. 9. A DA Form 1059 (Service School Academic Evaluation Report), issued by the 218th Regiment Noncommissioned Officer (NCO) Academy on 31 July 2014 confirms his successful attendance and completion of the Warrior Leader Course from 10 July to 1 August 2014. REFERENCES: Army Regulation 600-8-22 (Military Awards) states: a. The Noncommissioned Officer Professional Development Ribbon is awarded to Soldiers for successful completion of designated NCO professional development courses. Soldiers must successfully complete one or more of the courses listed in AR 350–1 (Army Training and Leader Development), chapter 3, which are further described on the Army Training Requirements and Resources System website. Table 5-1 describes the Noncommissioned Officer Education System (NCOES) creditable courses for award Noncommissioned Officer’s Professional Development Ribbon and devices. The basic ribbon is authorized for completion of the primary level of NCOES, the Warrior Leader Course. b. An oak leaf cluster is issued to denote award of second and succeeding awards of decorations (other than the Air Medal) and unit awards. c. Arabic numerals are issued instead of a medal or ribbon for second and succeeding awards of the Air Medal, Overseas Service Ribbon, NCO Professional Development Ribbon, and others. The ribbon denotes the first award and numerals starting with the numeral 2 denote the number of subsequent awards. DISCUSSION: 1. The applicant was awarded two awards of the Army Commendation Medal. His DD Form 214 incorrectly shows three awards. 2. The applicant was awarded two awards of the Army Achievement Medal. His DD Form 214 incorrectly shows only one award. 3. The applicant's unit in Iraq was awarded the Meritorious Unit Commendation. This award is not listed on his DD Form 214. 4. The applicant completed the Warrior Leader Course. He is authorized award of the Noncommissioned Officer Professional Development Ribbon, which is not recorded on his DD Form 214.
