BOARD DETERMINATION/RECOMMENDATION:

1.	The Board determined the evidence presented is sufficient to warrant a 
recommendation for partial relief. As a result, the Board recommends that all 
Department of the Army records of the individual concerned be corrected by 
amending his DD Form 214 (Certificate of Release or Discharge from Active 
Duty), for the period ending 8 August 2011 by:

   a.	deleting from item 12f (Record of Service – Foreign Service) the entry 
"0002  02  00" and replacing it with the entry "0003  01 28";

   b.	adding to item 18 (Remarks) the entry "SERVICE IN KUWAIT FROM 
20100301 – 20110228."

2.	The Board further determined the evidence presented is insufficient to warrant 
a portion of the requested relief. As a result, the Board recommends denial of so 
much of the application that pertains to the number of campaign stars, amount of 
time served in MOS, mailing address, and nearest relative.
