Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the ABCMR in Docket Number AR201000029366 on 23 June 2011. 2. The applicant's previous application for correction of his DD Form 214 for the period ending 26 March 2006 to show primary MOS 25C was granted by ABCMR in Docket AR201000029366, dated 23 June 2011. 3. He was ordered to active duty as a member of the Texas Army National Guard in support of Operation Enduring Freedom on 12 March 2005. 4. On 26 March 2006, he was released from active duty and returned to control of the Texas Army National Guard. 5. His DD Form 214 for the period ending 26 March 2006 shows he was awarded or authorized the: * Army Commendation Medal * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * Armed Forces Reserve Medal with "M" Device * Combat Action Badge * Afghanistan Campaign Medal 6. He provided his basic combat training individual training record showing he qualified as Expert with the hand grenade and as Marksman with the M-16 rifle. Awards of the Expert Marksmanship Qualification Badge with Grenade Bar and Marksman Marksmanship Qualification Badge with Rifle Bar (M-16) are not shown on his DD Form 214 for the period ending 26 March 2006. 7. His records show he was awarded the Afghanistan Campaign Medal for participation in Operation Enduring Freedom VI while his unit was attached to Combined Joint Task Force-76 during the period 25 March 2005 through 6 March 2006. This award confirms his period of service in Afghanistan. 8. He provided an Army Commendation Medal Certificate awarding him the Army Commendation Medal for the period 7 March 2005 through 6 March 2006. This award confirms deployed to Afghanistan with the 111th Area Support Group as a part of Combined Joint Task Force-76. 9. The Office of the Under Secretary of Defense for Personnel Readiness website published a table showing the JMUAs approved Department of Defense activities. It shows Headquarters, Combined Joint Task Force-76, Bagram Airfield, Afghanistan, was approved for award of the JMUA for the periods 16 March 2005 through 21 February 2006 and 22 February 2006 through 2 February 2007. 10. The applicant was assigned to Headquarters, Combined Joint Task Force-76 from 25 March 2005 through 6 March 2006 during two periods for which the JMUA was approved. The JMUAs are not shown on his DD Form 214 for the period ending 26 March 2006. BOARD DISCUSSION: After reviewing the application and all supporting evidence, the Board found that full relief was warranted. 1. There is sufficient evidence to indicate that the applicant’s most recent DD Form 214, ending 26 March 2006, and DD Form 215, dated 23 September 2011, are absent important information regarding the applicant’s service. The record indicates that the applicant is authorized badges and unit awards not reflected on his current discharge documents 2. The Board also found that because the applicant already has both two DD Forms 214 and a DD Form 215, issuing another DD Form 215 would be confusing and cumbersome. Therefore, the board found that issuing a new DD Form 214 is appropriate. 3. The Board discussed that the new DD Form 214 would reflect his MOS 25C and his dates of service, because they are already reflected on his DD Form 215 dated 23 September 2011, which will be incorporated into the new DD Form 214. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of Army records of the individual concerned be corrected by issuing the applicant a new DD Form 214 by: a. combining data from the following: * the most recent DD Form 214 for period ending 26 March 2006, * the most recent DD Form 215, dated 23 September 2011, and b. adding the following authorized awards and badges: * Joint Meritorious Unit Award (2nd Award) * Marksman Marksmanship Qualification Badge with Rifle Bar (M-16) * Expert Marksmanship Qualification Badge with Grenade Bar 2. The Board recommends voiding out the applicant’s DD Form 214 ending 26 March 2006 and DD Form 215 dated 23 September 2011 after they are combined into the new DD Form 214.
