The applicant states item 7a (Place of Entry into Active Duty) on his DD Form 214 is incorrect. It should say El Paso, Texas. 2. The applicant provides his enlistment contract, DD Form 4/1. Item 4 (Place of Enlistment/Reenlistment) states "El Paso MEPS, El Paso TX". This document was completed on 27 April 2007. The applicant's DD Form 214 shows in item 7a "El Monte, California." 3. Army Regulation 635-8 (Separation Documents and Processing) establishes the standardized policy for preparing and distributing the DD Form 214. The source document used to complete item 7a of the DD Form 214 is the initial enlistment contract or order to active duty. BOARD DISCUSSION: The Board considered the applicant’s request with all supporting documents, evidence in the service record, applicable regulations, policy, and guidance. The applicant’s enlistment contract, DD Form 4/1 item 4 (Place of Enlistment/Reenlistment) states "El Paso MEPS, El Paso TX". Army Regulation 635-8 (Separation Documents and Processing) directs the source document used to complete item 7a of the DD Form 214 is the initial enlistment contract or order to active duty. Therefore the Board finds the applicant’s request is warranted.
