The applicant states, in effect, the incorrect date overlaps with a previously issued DD Form 214 with an ending date of 30 December 2011. 2. Evidence shows the applicant entered active duty as a member of the U.S. Army Reserve on 3 June 2011. The DD Form 214 he was issued shows he was honorably released from active duty on 30 December 2011, at the completion of his required active duty service. 3. Order Number 007918 issued by 81st Regional Support Command on 25 October 2012, ordered him to active duty for 71 days effective 1 November 2012. 4. Orders A-12-221392 issued by U.S. Army Human Resources Command on 4 December 2012, ordered him to active duty for 365 days effective 11 January 2013. 5. Order Number 006792 issued by 81st Regional Support Command on 13 November 2013, ordered him to active duty for 181 days effective 11 January 2014. This order was subsequently amended to extend his period of active duty to 263 days. 6. The applicant was honorably released from active duty at the completion of his required active duty. The DD Form 214 he was issued shows in item 12a that he entered active duty on 1 November 2011. He was released from active duty on 30 September 2014. 7. During the processing of this case, on 21 May 2019, an advisory opinion was obtained from the Chief, Personnel Management Division, U.S. Army Reserve Command. The advisory official stated that there was sufficient evidence to grant full relief. She further stated the applicant’s DD Form 214 for the period of active duty ending on 30 September 2014 should be adjusted to read the date entered active duty as 1 November 2012. 8. The standardized policy for preparing and distributing the DD Form 214 states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It further states that for item 12a, data sources are enlistment contract(s), initial order to active duty, active duty order, previously issued DD Forms 214, DA Form 1506 (Statement of Service – For Computation of Length of Service for Pay Purposes), and a complete review of the Soldier’s official record. Enter the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued. BOARD DISCUSSION: The Board agreed that the evidence confirms the applicant's DD Form 214 for the period ending 30 September 2014 shows an incorrect entry date. The Board determined the DD Form 214 in question should be corrected to show he entered active duty on 1 November 2012.
