THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 9 (Command to Which Transferred) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was transferred to the 180th Transportation Company vice the 415th Civil Affairs Battalion, in Muskegon, MI.

2.  The applicant states his DA Form 3540 (Certificate and Acknowledgement - U.S. Army Reserve - Service Requirements and Methods of Fulfillment) stated he would be assigned to the 180th Transportation Company after graduating from advanced individual training (AIT).  The DD Form 214 has the wrong unit listed. 

3.  The applicant provides his DA Form 3540.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the U.S. Army Reserve on 25 March 2016. 

	a.  Item 32 (Specific Option/Program Enlisted For, Military Skill, or Assignment to a Geographical Area Guarantees) of his DD Form 1966 (Record of Military Processing) shows he enlisted for assignment to the 180th Transportation Company, 1430 Parslow Drive, Muskegon, MI 49441. 

	b.  Section IV (Service Obligation) of his DA Form 3540 shows he enlisted for the following Reserve unit:  180th Transportation Company, 1430 Parslow Drive, Muskegon, MI 49441.

2.  He entered active duty for training (ADT) on 9 May 2016.  He completed required training at Fort Leonard Wood, MO, for award of military occupational specialty 88M (Motor Transport Operator). 

3.  He was honorably released from ADT on 2 September 2016.  Item 9 of his DD Form 214 shows he was transferred to the 415th Civil Affairs Battalion, 1430 Parslow Drive, Muskegon, MI 49441.

4.  On 23 April 2017, he was promoted to private first class.  His promotion DA Form 4187 (Personnel Action) was signed by the Commander, 180th Transportation Company, 1430 Parslow Drive, Muskegon, MI 49441.

REFERENCE:

Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management.  Chapter 5 states the DD Form 214 is a summary of the Soldier’s most recent period of continuous active duty.  It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge.  Paragraph 5-6 prescribes the rules for preparing the DD Form 214.  

	a.  Block 9 pertains to the command to which transferred.  For entry determinations see table 5–1 and enter the applicable location dependent on the Soldier’s status on termination of active duty.

	b.  Table 5-1, Rule 2, when the separation type is a release from ADT (includes initial ADT other than by discharge), and the Soldier's status is to return to the U.S. Army Reserve troop program unit (TPU), then enter the designation of the TPU from which entered active duty. 

DISCUSSION:

The applicant enlisted for assignment to the 180th Transportation Company.  Following completion of AIT and award of an MOS, his DD Form 214 incorrectly lists the command to which transferred in item 9.
