The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Army Commendation Medal to his DD Form 214 for the period ending 15 March 2015. 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending on 15 March 2015 to show in: * Item 7b (Home of Record (HOR) At Time Of Entry) – "XXX Upper Ridge, Alpine, WY 83128" instead of "XXX Trail Ridge, Alpine, WY 83128" * Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) – Army Commendation Medal 2. The applicant states his correct HOR is "XXX Upper Ridge, Alpine, WY 83128." He was awarded the Army Commendation Medal after his discharge by Permanent Orders (PO) Number 155-001 on 4 June 2015. 3. The applicant provides his DD Form 214, a DA Form 638 (Recommendation for Award), and Army Commendation Medal Certificate. CONSIDERTION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 1 June 2010. 2. Item 5 of his DD Form 1966 /1 (Record of Military Processing – Armed Forces of the United States) and item 3 (HOR) of his DD Form 4/1 (Enlistment/ Reenlistment Document – Armed Forces of the United States) lists his HOR as "XXX Trail Ridge, Alpine, WY 83128." 3. He was honorably discharged on 4 October 2010 to accept appointment as a warrant officer in the Army. His DD Form 214 shows he completed 4 months and 4 days of active service. His DD Form 214 lists in item 7b – "XXX Trail Ridge, Alpine, WY 83128." 4. He was appointed in the U.S. Army Reserve, as a chief warrant officer one, on 5 October 2010 and entered active duty on the same date. He completed training and was awarded military occupational specialty 153M (UH-60M Pilot). He was deployed and served in Afghanistan from 29 April 2013 to 29 April 2014. 5. He was honorably retired on 15 March 2015 by reason of permanent disability. His DD Form 214 shows he completed 4 years, 5 months, and 11 days of active service. His DD Form 214 lists in: * Item 7b – "XXX Trail Ridge, Alpine, WY 83128" * Item 13 – no entry for the Army Commendation Medal 6. He provides the following: * DA Form 638, dated 6 May 2015, recommending him for award of the Army Commendation Medal for exceptionally meritorious service for the period from 15 August 2012 to 9 March 2015; the award was approved by PO Number 155-001 on 4 June 2015 * Army Commendation Medal Certificate for the award announced in PO Number 155-001 REFERENCES: Army Regulation 635-5 (Separation Documents), in effect at the time, provided item-by-item instructions for completing a DD Form 214, including item 7b. It provided that a Soldier’s initial enlistment contract or appointment document is the source for this data. It also noted that the HOR was the place recorded as the HOR of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty. The HOR could not be changed unless there was a break in service of at least 1 full day in accordance with Joint Federal Travel Regulations, Volume 1, Appendix A, Part I. DISCUSSION: 1. PO awarded the applicant the Army Commendation Medal on 4 June 2015. His DD Form 214 does not list this award. 2. The HOR is the address the Soldier identified as his home when he entered active duty. By regulation, the HOR cannot be changed unless the individual had a break in service of at least 1 full day. There is no evidence he had a break in service of at least 1 full day from his appointment date on 5 October 2010 to his separation date on 15 March 2015. As such, there is no error in the HOR listed in item 7b of his 2015 DD Form 214. 3.
