The applicant states there was an administrative error with the recording of the mailing address and nearest relative. 2. Review of the applicant's service records shows: a. Having had prior service in the Army National Guard and Regular Army, the applicant enlisted in the RA on 20 August 2008. He served in a variety of assignments. b. On 8 May 2014, a physical evaluation board (PEB) convened and found his medical conditions rendered him physically unfit. The PEB recommended his permanent retirement. c. He retired on 17 August 2014 due to disability. His DD Form 214 shows in: * Block 19a, the mailing address as "1XXX E X ST AVE, Broomfield, Colorado, 80020" * Block 19b, the name of his nearest relative and the address "1XXX E X ST AVE, Broomfield, Colorado, 80020" 3. By regulation (AR 635-8), the DD Form 214 is a summary of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of separation from active duty. The DD Form 214 is not intended to have any legal effect on termination of a Soldier’s service. Paragraph 5-6 provides for detailed instructions for data required in each block of the DD Form 214. Item 19 provides contact address information. For item 19b, the data source is as provided by the Soldier; the name and address of a relative should be someone who will know Soldier’s location and address at all times. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was not warranted. The applicant’s contentions were carefully considered. The mailing address reflected on the DD Form 214 was used at the time of his separation. The Board agreed records must reflect the conditions and circumstances that existed at the time the records were created. In the absence of evidence that shows a material error or injustice, there is a reluctance to recommend that those records be changed.
