The applicant states when he was out-processing, they did not have his previous DD Form 214 on record. He now provides this form and would like Blocks 12d (Total Prior Active Service) and 12e (Total Prior Inactive Service) corrected. 2. Review of the applicant's service records shows: a. He enlisted in the U.S. Army Reserve (USAR) on 22 July 1997. b. He entered active duty for training (ADT) on 22 July 1997 and completed medical training for award of military occupational specialty 91D (Operating Room Specialist). c. He was honorably released from active duty on 28 October 1998 and transferred to his Reserve unit, 73rd Medical Hospital. His DD Form 214 shows he completed 1 year, 3 months, and 7 days of active service. d. He reenlisted in the USAR on 15 November 2003. He was honorably discharged from the USAR on 15 November 2006. e. After a break, he again enlisted in the USAR on 25 March 2010. f. He entered active duty on 4 February 2016 and served in Iraq from 22 March to 12 December 2016. He was honorably released from active duty on 9 January 2017. His DD Form 214 for this period of service shows in: * Block 12a (Date Entered Active Duty This Period) - 2016-02-04 * Block 12b (Separation Date This Period) - 2017-01-19 * Block 12c (Net Active Service This Period) - 0000-11-16 * Block 12d (Total Prior Active Service) SEE Block 18 (Remarks) * Block 12e (Total Prior Inactive Service) SEE Block 18 * Block 18 (Remarks), in part, A DD Form 215 will be issued for the missing information g. On 13 January 2017, the applicant was issued a DD Form 215 that corrected: * Block 12b (Separation Date This Period) - from 2017-01-19 to 2017-01-22 * Block 12c (Net Active Service This Period) - from 0000-11-16 to 0000-11-19 h. The applicant's service record is void of a DA Form 1506 (Statement of Service for Computation of Length of Service or Pay Purposes) which would have captured his inactive service and documented a member's request for verification of military service. 3. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active and prior inactive duty service at the time of release from active duty, retirement, or discharge. AR 635-8 states for * Block 12d, Total Prior Active Service. From previously issued DD Form 214, DD Form 220, DA Form 1506, or Retirement Point Annual Statement, enter total amount of prior active military service less lost time, if any. If not applicable, enter “00-00-00” * Block 12e: Total Prior Inactive Service. From previously issued DD Form 214, Retirement Point Annual Statement, DA Form 1506, or member’s record brief, enter the total amount of prior inactive service, less lost time, if any. Inactive Service should be annotated in block 12e and block 18 as prior inactive service entered in format “0000 00 00” 4. The Administrative Notes of this Record of Proceedings may be used to correct his prior active service, based on his previous DD Form 214. However, the processing of his DA Form 1506 (for inactive service) may be done at the unit level or at the Regional Support Command/Regional Readiness Command level. Army Regulation 15-185 (ABCMR), the regulation under which this Board operates, states that the Board will not consider any application if it determines that an applicant has not exhausted all administrative remedies available to that applicant. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was warranted. Based upon the applicant having prior active duty service that is not currently reflected on his DD Form 214, dated 19 January 2017, the Board concluded that there was sufficient evidence to show a correction was warranted.
