The applicant did not file within the three year time frame provided in Title 10, United States Code (USC), section 1552 (b); however, the Army Board for Correction of Military Records conducted a substantive review of this case and determined it is in the interest of justice to excuse the applicant's failure to timely file. 2. The applicant states the places he was stationed (Fort Leonard Wood, MO, Fort Gordon, GA, and Kaiserslautern, Germany are not on his DD Form 214. 2. The applicant provides: a. His DA Form 20, page 3, showing his duty assignments. b. His DD Form 214 showing his service from 16 March 1964 to 1 March 1967. 3. The review of the applicant’s service record shows: a. He enlisted in the Regular Army on 16 March 1964. b. His DA Form 20 shows he was assigned to: * Charlie (C) Company, 5th Battalion, 3rdTraining Regiment, Fort Leonard Wood, MO on 16 March 1964 to 15 June 1964 * Student Company Victory (V), Fort Gordon, GA 16 June 1964 to 19 September 1964 * Bravo (B) Company, 1st Signal Battalion, Germany from 20 September 1964 to 2 June 1965 * Alpha (A) Company, 440th Signal Battalion, Germany from 3 June 1965 to 27 February 1967 4. He was honorably released from active duty on 1 March 1967, under provisions of AR 635-200, SPN 411 – early separation of overseas returnee. His DD Form 214 shows his service characterized as under honorable conditions. He completed 2 years, 11 months and 12 days of active service this period. He had 4 days lost time from 22 July 1966 to 25 July 1966. 5. By regulation, AR 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214 and DD Form 215. In pertinent part, it states the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty. There was no regulatory requirement to list duty assignments on the DD Form 214, except for item 12 (Last Duty Assignment and Major Command) BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was not warranted. The applicant’s contentions were carefully considered. The Board agreed regulatory guidance provides for the listing of the last unit of assignment on the DD Form 214 only; not each duty assignment to which the member was assigned. The Board determined there was insufficient evidence of an error or injustice in this case which would warrant correction. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214 and DD Form 215. In pertinent part, it states the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. Once a DD Form 214 has been issued, do not reissue, except when two DD Forms 215 have been issued and an additional correction is required. There was no regulatory requirement to list duty assignments on the DD Form 214, except for item 12 (Last Duty Assignment and Major Command). 2. The DA Form 20 is a snapshot of a Soldier's personnel data as it was at the time the form was produced and it was used as an internal management tool to assist officials processing a Soldier's assignment/reassignment, promotions, and career progression.
