The applicant states his DD Form 214 has the wrong discharge date. He provides: * Orders 179-070, dated 28 June 2013, issued by Joint Force Headquarters IN, Indianapolis, IN, ordered the applicant to full time NG duty from 1 July 2013 – 30 September 2013 * Summary of retired pay account, dated 16 December 2015, shows the applicant’s retirement date as 27 November 2015 2. A review of the applicant’s service record shows: a. He enlisted in the Army National Guard (ARNG) on 20 November 2009. b. His DD Form 214s for periods 16 February – 4 June 2010, 28 September 2011 – 4 November 2012, and 1 July – 30 September 2013 show he was honorably released from active duty for completion of required active service. c. His informal physical evaluation board proceedings, date board convened was 30 September 2015, shows the applicant is physically unfit and recommended a rating of 50% and the applicant’s disposition to be permanent disability retirement. d. Orders D296-34, dated 23 October 2015, issued by the U.S. Army Physical Disability Agency, Arlington, VA, shows he was released from assignment and duty because of physical disability incurred while entitled to basic pay and under conditions that permit his retirement for permanent physical disability. He was placed on the retired list on 27 November 2015. e. Orders 302-1024, dated 29 October 2015, issued by Joint Force Headquarters IN, Indianapolis, IN, assigned the applicant to the Retired Reserve, Fort Knox, KY, on 26 November 2015. f. His NGB Form 22 (NG Report of Separation and Record of Service), shows he was honorably separated from active duty on 26 November 2015, under the provisions of NG Regulation 600-200 (Enlisted Personnel Management), paragraph 6-36s, placement on the permanent disability retired list. He completed 6 years and 7 days of net service this period. His NGB Form 22 shows in: * block 4 (Date Of Enlistment), 20091120 * block 18b (Effective Date), 20151126 4. Army Regulation 635-8 (Separation Processing and Documents) states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. There are no provisions allowing for the addition to the DD Form 214 of achievements or transactions accomplished subsequent to the period covered by the DD Form 214. Soldiers who are also leaving ARNG Service will be issued separation orders and an NGB Form 22 (National Guard Report of Separation and Record of Service) by the Soldier’s State Joint Forces Headquarters. 5. NGR 600-200 (Enlisted Personnel Management) establishes standards, policies, and procedures for the management of the ARNG and the ARNG Guard of the United States enlisted Soldiers for classification and reclassification, personnel management, assignment and transfer, including interstate transfer, special duty assignment pay, enlisted separations, and the command sergeant major program. Table F-1 (Procedures for issuing NGB Form 22, Report of Separation and Record of Service), states for block 4 (Date of Enlistment), self-explanatory-rom latest DD Form 4 and block 18b (Effective Date), expired term of service or effective date of discharge order BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was not warranted. Based upon the documentary evidence in the record, the Board found a DD Form 214 with the requested dates already noted; the service credit requested is already captured on a DD Form 214. As a result, the Board concluded there was insufficient evidence of a need for correction to the service record.
