Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20070010346 on 20 December 2007. 2. The applicant states his birth name should be amended to his birth given name or that there be a reference that and are one and the same person belonging to his social security number. During the induction process he was not advised to use his birth name per Army regulation. 3. The applicant provides: a. Self-authored statement, dated 8 May 2017, requesting his name be changed from to . It was brought to his attention that his birth name should have been used. The Catholic nuns changed his name from Fxxxx for for J . He would like all of his affairs under his birth given name. He served in a period where many ran or differed which means he completed his civic duty. b. Certificate of Live Birth, reflecting the name 4. By regulation, DA Form 20 (Enlisted Qualification Record) is the basic source document for preparing the DD Form 214. 5. A review of the applicant’s service records shows the following: a. 5 July 1972, he was inducted into the Army of the United States. b. The following documents show his name as Fxxxx X. H : * DD Form 398 (Statement of Personal History) * DD Form 47 (Record of Induction) * DD Form 20 * DD Form 214 c. 27 June 1974, he was honorably released from active duty. 6. The applicant’s record contains a previous ABCMR case in which he petitioned the Board for a name change, the Board denied his request on 20 December 2007. BOARD DISCUSSION After reviewing the Record of Proceedings and all supporting documents, the Board determined that relief was not warranted. Because the service record had no record with the requested name and reflect circumstances as they existed at the time of service, the Board found no relief was appropriate. The Board also stated that placing these records in the OMPF of the service member will help remove any confusion of the different names of the applicant, if needed. REFERENCE Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents that are furnished to individuals who are retired, discharged, or released from active military service. Separation documents provide the recipient with a record of their military service. DA Form 20 (Enlisted Qualification Record) is the basic source document for preparing the DD Form 214. However, in preparing the DD Form 214, personnel officers should review all available documents such as the Military Personnel Records Jacket, Military Personnel Financial Data Records, Personal Financial Record, reassignment orders, enlisted or induction records and any DD Forms 214 for prior service.
