The applicant states, he is legally separated from Axxxxx Mxxxxx and they are in the process of getting divorced. He is currently living at XXX Pxxxxxx Dxxx Greenburg XX 1XXXX. 2. A review of the applicant’s service records shows the following: a. He enlisted in the Regular Army on 28 January 2009. b. His Enlisted Records Brief reflects his mailing address in Section IV (Personal/Family Data) in block 19 as XXX BXXXXX Dxxx, RaXXXff KXXXXXX 4XXXX. c. On 27 February 2015, he received an honorable discharge. His DD Form 214 shows in Block 19a, the mailing address XXX Mxxxxxx D. Xxxxx Xxx Xxxx 1XXXX and in Block 19b, the name of the nearest relative as Axxxxx Mxxxxxx. 3. By regulation (AR 635-5), the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation. Items 19a shows the address after separation and item 19b shows the name and address of the member's nearest relative as provided by the Soldier. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was not warranted. The applicant’s contentions were carefully considered. The contested addresses were changed after the period of service covered by the DD Form 214. The Board agreed there is no error as records must reflect the conditions and circumstances that existed at the time the records were created. Paragraph 2-4h (19) Mailing Address After Separation and Nearest Relative will be provided by the Soldier. a. This address must be a permanent address. Civilian penal institutions will not be accepted. b. Advise a Soldier that the name and address of a relative should be someone who will know their location and address at all times. When a relative is not available, use a close friend.
