The applicant states Block 12a of her DD Form 214 should reflect the date entered active duty as 1 August 2005 vice 11 December 2006. Her DD Form 214 was completed after she had permanently changed station. She was mailed this form in error. 2. The applicant provides her DD Form 214 and 22 sets of orders and/or amendments. 3. Review of the applicant's service records shows: a. She executed an oath of office and was appointed as a Reserve commissioned officer on 27 September 1993 in the grade of second lieutenant. She completed the Army Medical Department Officer Basic Course. b. She served in a variety of assignments and she was promoted to major in the U.S. Army Reserve in February 2003. She was assigned to a troop program unit (TPU) in Germany (Medical Support Unit Europe, Wurzburg/Heidelberg. c. On 25 July 2005, Headquarters, 7th Army Reserve Command, Germany, ordered the applicant deployed on temporary change of station (TCS) in support of Operation Iraqi Freedom, proceedings on or about 17 August 2005 for 545 days. d. It is unclear if she entered active duty or deployed in support of Operation Iraqi Freedom. Likewise, it is equally unclear if she demobilized and/or was issued orders releasing her from active duty. e. On 28 December 2006, Headquarters, 7th Army Reserve Command, Germany, ordered the applicant to active duty as a member of her Reserve unit in support of mobilization, reporting on or about 13 January 2006, for 185 days. f. Several amendment orders, revocation orders, and new orders were issued by either Headquarters, 7th Army Reserve Command, or the U.S. Army Human Resources Command, for a variety of reasons, and under different statutory authorities: * Deployment in support of Operation Iraqi Freedom/Enduring Freedom * TTAD (Temporary Tour of Active Duty) support * Contingency Operation for Active Duty Operational Support h. On 9 December 2006, Orders T-05-2006-0006A01, amended Orders T-05-2006- 0006A, dated 25 July 2005, to show her report date as 11 December 2006. i. She entered active duty on 11 December 2006. She served in a variety of assignments, via an extensive number of orders and amendments. j. On 30 October 2014, Installation Management Command - Europe published Orders 273-0002 honorably releasing her from active duty effective 30 September 2014, and transferring her back to the U.S. Army Reserve Control Group (Reinforcement). k. She was honorably released from active duty on 30 September 2014. Her DD Form 214 shows in: * Block 12a (Date Entered Active Duty This Period), 2006-12-11 * Block 12b (Separation date This Period) - 2014-09-30 * Block 12c (Net Active Service This Period), 0001-00-00 l. Two emails were sent to the Defense Finance and Accounting Service (DFAS) to verify the exact date the applicant entered active duty. DFAS did not respond. m. The applicant's service record is void of a DA Form 1506 (Statement of Service – For Computation of Length of Service for Pay Purposes) that can help establish her date of entry 3. By regulation (AR 635-8), Block 12a, Date Entered Active Duty This Period, the data sources are enlistment contract(s), initial order to active duty, active duty order, previously issued DD Forms 214, DA Form 1506 (Statement of Service – For Computation of Length of Service for Pay Purposes), and a complete review of the Soldier’s official record. Enter the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued. Do not depend on basic active service date for this data item. The basic active service date can be an adjusted date. * in the absence of a verification of date of entry from DFAS, the applicant's DA Form 1506 (Statement of Service) together with her Leave and Earnings Statement may establish her date of entry * in the absence of her date of entry, her net active service for the period cannot be computed accurately BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board determined that relief was not warranted. Based upon the original orders of the applicant being amended with a new report date (the date reflected on the applicant’s DD Form 214), the Board concluded there was insufficient evidence of an error or injustice which would warrant a correction to the applicant’s record.
