The applicant states another person’s information is in her file. The JMUA for service in the Pacific Command, dated 20 February 2015, does not belong to her. She has never served under the Pacific Command and the social security number (SSN) on the document is not hers, but very close in number. 2. The applicant provides Joint Staff Permanent Order Number J-ISO-0023-15, dated 13 February 2015, which states the Chairman of the Joint Chiefs of Staff awarded the JMUA for exceptionally meritorious service to Headquarters, United States Pacific Command and its subordinate units during the period of 10 March 2012 to 20 February 2015. The last four of the SSN found on the document and its accompanying citation are XXX-XX-XXXX and the applicant’s last four are XXX-XX-XXXX. 3. A review of the applicant’s service record shows: a. She was appointed as a Reserve commissioned officer on 22 July 1992 and executed an oath of office on 3 August 1992. b. The JMUA is reflected on her DA Form 2-1 (Personnel Qualification Record), her Officer Record Brief, and her DD Form 214 (Certificate of Release or Discharge from Active Duty). c. On 29 December 2016, she was released from active duty with an honorable characterization of service. Her DD Form 214 shows she completed 7 years, 2 months, and 9 days of total active service with 13 years, 10 months, and 17 days of prior inactive service. 4. By regulation, the DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was warranted. The applicant’s contentions were carefully considered. Based upon the documentary evidence provided by the applicant and found within the military service record, the Board agreed the JMUA was clearly erroneously added to her OMPF and recommends relief by removing the document.
