ARMY BOARD FOR CORRECTION OF MILITARY RECORDS RECORD OF PROCEEDINGS IN THE CASE OF: BOARD DATE: 17 October 2019 DOCKET NUMBER: AR20170014792 APPLICANT REQUESTS: correction of his DD Form 214 (Armed Forces of the United States (U.S.) Report of Transfer or Discharge) by changing the location for the Army Training Center (ATC) from Fort Gordon, GA to Fort Polk, LA. APPLICANT'S SUPPORTING DOCUMENTS CONSIDERED BY THE BOARD: * DD Form 149 (Application for Correction of Military Record) * DD Form 214 FACTS: 1. The applicant did not file within the three year time frame provided in Title 10, United States Code (USC), section 1552 (b); however, the Army Board for Correction of Military Records conducted a substantive review of this case and determined it is in the interest of justice to excuse the applicant's failure to timely file. 2. The applicant states he would like for the ATC location to be corrected. 3. A review of the applicant’s service record shows: a. He was inducted in the Army of the United States on 21 March 1968. He completed the 8-week Wireman Course at Fort Polk, LA and was awarded military occupational specialty (MOS) 36K (Wireman) on 26 July 1968. b. His DA Form 20 (Enlisted Qualification Record), block 35 (Record of Assignments) and Special Orders Number 121, issued on 21 May 1968, by Headquarters, U.S. Army Training Center (USATC), Infantry (INF), Fort Benning, GA, show the applicant was assigned to the USATC INF at Fort Polk, LA for MOS training. c. He was honorably released from active duty on 14 March 1970 and was transferred to the U.S. Army Reserve Control Group (Annual Training) MO to complete his Reserve obligation of 25 March1974. d. His DD Form 214 shows he completed 1 year, 11 months, and 19 days of net service. Block 25 (Education and Training Complete), shows USATC FT Gordon GA Wireman, 8 weeks. 5. By regulation (AR 635-5), the DD Form 214 provides the individual with documentary evidence of his or her military service. It is important that information entered on the form should be complete and accurate. It states for item 25, enter service schools, including major courses which were successfully completed, and military sponsored courses completed in civilian schools and colleges during period covered by the DD Form 214 being prepared. This entry includes the highest civilian education level acquired during this period of military service, if appropriate. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found relief was warranted. The applicant’s records show he went to basic training and MOS 36K at Fort Polk. His DD Form 214 inadvertently listed this training at Fort Gordon. Based upon the apparent error, the Board voted to grant relief. It states for item 25, enter service schools, including major courses which were successfully completed, and military sponsored courses completed in civilian schools and colleges during period covered by the DD Form 214 being prepared.
