The applicant states he would like to change his name on his military documents. His DD Form 214 (Certificate of Release or Discharge from Active Duty) item 1 (Last Name, first Name, Middle Initial) should reflect his legal name of 2. The applicant provides a copy of his birth certificate, which shows his legal name as . 3. A review of the applicant’s service record shows: a. He enlisted in the Regular Army on 1 July 2010. The general instructions stated all available records would be used as a basis for preparation of the DD Form 214. The specific instructions for item 1 (Last Name, First Name, Middle Name) stated to enter the last name, first name, and full middle name or names, if any. b. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation. BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board found the relief was warranted. The applicant’s contentions were carefully considered. He did not use the contested name listed on his DD Form 214 when he initially entered military service. His enlistment document shows the name listed on his birth certificate. The Board agreed there is sufficient evidence to grant relief.
