The applicant states there are six total assignments missing on his DD Form 214. There are gaps in the timeline on his form. The most recent deployments/assignments are on his DD Form 214, but the ones from 21 September 1994–30 April 1997 (Headquarters and Headquarters Battery (HHB) 35th Air Defense Artillery (ADA) Brigade (BDE) Fort Lewis, WA), 21 July 1996–15 December 1996 (Prince Sultan Air Base, Saudi Arabia), 1997-1999 (Kaiserslautern, Germany), 21 June 2001–1 November 2001 (Prince Sultan Air Base, Saudi Arabia), 11 March 2004–8 March 2005 (Wightman NCO Academy, Korea), and 10 February 2006–31 July 2008 (Kadena Air Base, Japan). He needs these deployments/assignments on his DD Form 214 to complete it. Until his DD Form 214 is completed he cannot submit his open burn pit registry questionnaire. 2. A review of the applicant’s service records shows the following: a. He enlisted in the Regular Army on 21 April 1994. He served through multiple reenlistments, in a variety of stateside or overseas assignments, and he attained the rank of master sergeant. b. His Enlisted Record Brief shows in Section I (Assignment Information): * 17 October 2014-16 October 2015 Qatar 12 months Operational * 28 December 2011-10 December 2012 Qatar 12 months Combat * 26 September 2006-8 August 2008 Japan 23 months Long Tour * 27 August 2005-15 October 2005 Egypt 2 months Combat * 9 March 2004-7 March 2005 Korea 12 months Short Tour * 26 January 2003-30 April 2003 Jordan 3 months Combat c. He was honorably retired on 31 May 2017. He completed over 23 years of active service. His DD Form 214 shows in: * Block 12f (Foreign Service) 5 years, 2 months, and 19 days * Block 18 (Remarks), "Service in Jordan from 26 January 2003-30 April 2003," "Service in Egypt from 27 August 2005-15 October 2005," and "Service in Qatar from 28 December 2011-10 December 2012 and 17 October 2014- 16 October 2015" 4. By regulation, AR 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of his or her military service. There is no provisions to list each specific assignment on the DD Form 214. However: * block 12f shows the total amount of foreign service completed during the period covered by the DD Form 214 * block 18 for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "Service in (Name of Country Deployed) From YYYYMMDD - YYYYMMDD)" BOARD DISCUSSION: After reviewing the application and all supporting documents, the Board determined partial relief was warranted. He deployed to Saudi Arabia twice. His Qatar, Jordan, and Egypt are listed. He deployed to Southwest Asia with Task Force 2-1 ADA from 13 July 1996-15 December 1996. Southwest Asia with Task Force 1-7 ADA from 12 June 2001-1 November 2001. The applicant’s service in Germany and Korea are captured in the Overseas Service block but neither was a deployment. His achievement medal specifically mentions his service in SWA. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected amending his DD Form 214 for the period ending 31 May 2017 by entering in block 18 "Service n Southwest Asia from 13 July 1996-15 December 1996 and 12 June 2001-1 November 2001.” 2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to listing his service in Germany, Korea, or any stateside assignments in Block 18.
