APPLICANT REQUESTS: that his DD Form 214 (Report of Separation from Active Duty) reflect the correct social security number (SSN) Xxx-Xx-XxxX.

APPLICANT'S SUPPORTING DOCUMENTS CONSIDERED BY THE BOARD:

   * Copy of DD Form 214 FACTS:
1. The applicant states he is requesting that his DD Form 214 be corrected to reflect (item 3) SSN Xxx-Xx-XxxX. Currently, his DD Form 214 (item 3) shows Xxx-Xx-XxxX. He further states that every time he had to recite his SSN in Oklahoma, they told him his SSN was incorrect since they were going by his DD Form 214. He kept requesting the Army to change it to the correct SSN but it never happened.

2. The applicant provides a copy of his SSN card that shows SSN Xxx-Xx-XxxX and a copy of his DD Form 214 which shows (item 3) 3xx-5x-3xx6.

3. A review of the applicant’s service record shows the following:

   a. He enlisted in the Army National Guard (ARNG) on 14 July 1977. His DD Form 4 (Enlistment/Reenlistment) shows his SSN (item 2) Xxx-Xx-XxxX.

b. DA Form 2-1 (Personnel Qualification Record) shows SSN (item 2) 3xx-5x-3xx6.

   c. Orders 237-104, dated 7 December 1979, order the applicant to active duty for 20 months and 11 days report date is 24 January 1980. The applicant’s SSN is shown as 3xx-5x-3xx6.

   d. He was relieved from active duty on 3 February 1978. His DD Form 214 shows SSN (item 3) Xxx-Xx-XxxX.

4. Army Regulation (AR) 635-5 (Separation Documents) DD Form 215 (Correction to DD Form 214, Certificate of Release or Discharge from Active Duty).will be issued to provide missing information or to correct any information. Item 3 (SSN) transcribe Social Security account number from DA Form 2139 (Military Pay Voucher).
“DD Form 215 (Correction to DD Form 214, Certificate of Release or Discharge from Active Duty) will be issued to provide missing information.” DD Form 215 will be issued when the missing information becomes available.

5. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation.

BOARD DISCUSSION:

1.  After reviewing the application and all supporting documents, the Board found the relief was not warranted. Based upon the documentary evidence provided by the applicant and found within the military service record, the Board found that the SSN currently reflected on the DD Form 214 is the SSN used throughout the military service of the applicant. Without justification from the applicant as to why the number is different, the Board concluded there was insufficient evidence of an error or injustice which warrants a correction.

2.  The applicant is advised that a copy of this decisional document will be filed in his official military personnel file. This should serve to clarify any questions or confusion about the difference in the SSN recorded in his military records and the SSN on his Social Security card.
