The applicant, in effect, requests correction of her military record due to her recent marriage. 3. She entered the Regular Army on 2 September 2009 under the name of Jaz___ Aqu___ Peg___ as shown on her DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States). 4. On 25 December 2011, she reenlisted in the Regular Army. At that time, there was no record of a name change and she continued to serve under the name shown on her first DD Form 4. 5. On 26 April 2015, she was retired due to permanent disability. She was issued a DD Form 214 showing her name as shown on her 2009 DD Form 4. Her separation documents on file in her military personnel service record to include her Enlisted Record Brief also show her name as shown on her entrance DD Form 4. There is no record of a request to change her name due to marriage during her period of active federal service. 6. On 23 July 2018 the applicant married, changing her last name from Aqu___ Peg___ to Bo___. 7. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. The information entered thereon reflects the conditions as they existed at the time of separation and as required by regulation. BOARD DISCUSSION: After reviewing the application and all supporting evidence, to include the applicant’s statement, the Board determined relief was not warranted because the applicant changed her name after separation. The applicant should be made aware the record of these proceedings will be placed in her Official Military Personnel File (OMPF) to assist when questioned about the name differences. The rules for completing the DD Form 214 state for Block 1 (Name) to compare original enlistment contract or appointment order and review official records for possible name changes.
