The applicant states, in effect, her last name was legally changed from "P" TO "C." 2. The applicant enlisted in the regular Army on 2 February 1988. 3. The applicant's record contains a series of DD Forms 4 (Enlistment/Reenlistment Document) which show her last name as "C." * 5 August 1987 (initial enlistment) * 6 December 1991 (1st reenlistment) * 24 June 1994 (2nd reenlistment) * 24 January 1997 (3rd reenlistment) 4. The applicant married and her last name changed from "C" to "P" sometime after her third reenlistment. 5. The applicant's DD Form 214 for the period 2 February 1988 through 10 April 2001 shows she was discharged to accept a commission. This form recorded her last name as "P." 6. She was honorably retired from active duty on 30 April 2011. Her DD Form 214 records her last name as "P." 7. She provided a copy of her retiree identification card, which records her last name as "P." 8. The applicant provided the following documents: * State , Superior Court of County – Final Order, filed 8 May 2019, which changed her last name from "P" to "C" * State Driver's License, issued on 8 May 2019, which shows her last name as "C" * Two Social Security Administration documents, 9 May 2019, which show her last name as "C" 9. Army Regulation 635-5 (Separation Documents) establishes the policy for preparing and distributing the DD Form 214. The regulation directs that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement, or discharge. BOARD DISCUSSION: After review of the application and all evidence, the Board determined that there is sufficient evidence to grant relief. The board found sufficient evidence to grant the applicant’s request to change her name reflected on DD Form 214 because her record contained evidence that she used the name “KLC” on several DD Form 4 enlistment contracts throughout her career. ? As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 by changing her name in item 1 to “KLC” per her DD Form 4 dated 97 JAN 24.
