The applicant states she wishes to changed her last name on her DD Form 214 due to marriage and she included a copy of her marriage certificate. 2. The applicant was appointed as a Reserve Commissioned Officer of the Army on 8 September 1992. Her DA Form 71 (Oath of Office – Military Personnel) shows her last name at the time as W____. 3. On 1 August 2000, she was appointed as a Commissioned Officer in Regular Army. Her last name is listed on her DA Form 71 at the time as S____. 4. She was honorably retired after completing sufficient service for retirement on 31 December 2012. Her DD Form 214 (Certificate of Release or Discharge from Active Duty) lists her last name as S____. 5. There are no documents in the applicant’s available records showing her last name as B____. 6. The applicant provided a copy of a Florida State marriage certificate, which shows she was married on 4 May 2019. Her last name is listed on the form as S____ and the last name of her spouse is listed as B____. BOARD DISCUSSION: 1. At the time of her retirement in 2012, the applicant was serving under the last name S___, which was correctly recorded on her DD Form 214. She changed her last name to B___ when she married in 2019. The Board agreed that her post-retirement name change due to marriage is not evidence of an error, inequity, or injustice that would provide a basis for changing the last name recorded on her DD Form 214. 2. The applicant is advised that a copy of this decisional document will be filed in her Official Military Personnel File. This should serve to clarify any questions or confusion in regard to the difference between the last name recorded on her DD Form 214 and the last name she now uses. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents that must be prepared for Soldiers at the time of retirement, discharge, or release from active duty service or control of the Active Army. It established standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of a Soldier’s most recent period of continuous active service and it provides a concise record reflective of active service at the time of release from active duty, retirement, or discharge.
