The DD Form 214 provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge. FACTS: 1. The applicant states her name was legally changed on 11 July 2018. 2. Her DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States), dated 14 January 2010, shows she enlisted in the U.S. Army Reserve on 14 January 2010 for a period of 8 years under the Delayed Entry/Enlistment Program with a commitment to enlist in the Regular Army for a period of 5 years not later than 6 April 2010. Her name is shown as P____ O____ J____. 3. She enlisted in the Regular Army and entered active duty on 9 June 2010. 4. Her DD Form 4, dated 3 March 2014, shows she enlisted (in-service) in the Army National Guard on 3 March 2014 for a period of 1 year, 9 months, and 14 days. Her name is shown as P____ O____ J____. 5. Directorate of Human Resources, Military Personnel/Division, Joint Base Lewis- McChord, Orders 112-0026, dated 22 April 2014, released her from active duty effective 8 June 2014. Her name is shown as P____ O____ J____. 6. She was honorably released from active duty on 8 June 2014. Her DD Form 214 shows her name as P____ O____ J____. 7. She was honorably discharged from the Army National Guard effective 9 May 2016. Her National Guard Bureau Form 22 (Report of Separation and Record of Service) shows her name as P____ O____ J____. 8. Headquarters, Military Department, State of, Office of the Adjutant General, Orders , dated 11 May 2016, discharged her from the Army National Guard effective 9 May 2016. Her name is shown as P____ O____ J____. 9. The County District Court Order Changing Name, dated 11 July 2018, ordered her requested name change effective 11 July 2018. BOARD DISCUSSION: 1. After reviewing the application and all supporting documents, the Board found that relief is not warranted. 2. The Board noted the applicant served under the name recorded on her DD Form 214 throughout her military service. The Board found insufficient evidence of mitigating factors to support a recommendation to change an Army record that accurately reflects the information available at the time it was created. The Board determined the name on the applicant's DD Form 214 is not in error or unjust. 3. The applicant is advised that a copy of these proceedings will be filed in her Official Military Personnel File, which should serve to explain the difference between the name under which she served and the name she now uses.
