The applicant states, in effect, that due to divorce, she would like her last name changed. 2. The applicant enlisted in the Regular Army on 16 September 2011. Her DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) lists her current legal name (maiden name). 3. The applicant’s military records contain a series of documents, which show her name changed on the following dates: a. DA Form 5960, dated 1 March 2013, wherein she has annotated she is married but signed her maiden name. b. DA Form 199, dated 8 June 2014, which shows her last name changed from her maiden name to the contested name. 4. The applicant was honorably retired from Active duty on 23 September 2014. Her DD Form 214 reflects the contested name. 5. The applicant provides a Divorce decree, dated March wherein her name was restored from the contested name to her current legal name (maiden name), after the period of her military service. 6. Army Regulation 635-5 (Separation Documents), dated 15 September 2000, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. For item 1, enter name in all capital letters. Include "Jr,"Sr," or "II" when needed, at the time of separation. This regulation also states that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement, or discharge. 7. For historical purposes, the Army has an interest in maintaining the integrity of its records. The data and information contained in those records should reflect the conditions and circumstances that existed at the time the records were created BOARD DISCUSSION: 1. The Board carefully considered the applicant’s request, supporting documents and evidence in the records. The Board considered the applicant’s statement, her record of service, her in-service name changes, the date of her separation and the date of the court order she provided showing her current name. The Board determined that the applicant’s requested name change was effective after her period of service. Based on a preponderance of evidence, the Board determined that the name reflected on the applicant’s DD form 214 was not in error or unjust. 2. The applicant is advised that a copy of these proceedings will be placed in her official records to clarify the difference in the name appearing in her records and the name she uses today. 3. After reviewing the application and all supporting documents, the Board found that relief was not warranted. ?
